曲安奈德曲池穴位注射对变应性鼻炎模型大鼠的影响

摘　　要：目的:探讨曲安奈德曲池穴注射治疗变应性鼻炎的可能机制。方法:将50只Wistar大鼠随机分为5组,每组10只。正常对照组不予致敏和激发,其余4组大鼠经卵清蛋白溶液致敏和激发后,穴位组曲池穴注射35μL(0.35mg)曲安奈德;肌注组以等剂量曲安奈德臀部肌肉注射;生理盐水组曲池穴注射35μL生理盐水。每组注射1次,观察7天。每次卵清蛋白溶液滴鼻后30min内记录大鼠喷嚏、抓鼻和流涕情况。取鼻中隔黏膜显微镜下观察组织形态学变化。结果:穴位组大鼠行为学变化逐渐接近正常对照组水平(P>0.05),肌注组和生理盐水组则无明显改变,与模型对照组比较无显著性差异(P>0.05);组织形态学显示模型对照组、肌注组和生理盐水组鼻黏膜下组织间质水肿,小血管扩张,组织中嗜酸粒细胞浸润明显,纤毛紊乱。穴位组黏膜基本完整,未见嗜酸粒细胞浸润。正常对照组未见上述病理改变。结论:曲安奈德曲池穴注射疗法能够减轻变应性鼻炎模型大鼠鼻黏膜变应性炎症反应。Objective： To explore the therapeutic mechanism of triamcinolone acetonide injection into Quchi acupoint for allergic rhinitis. Methods： Fifty Wistar rats randomized to five groups of 10 rats each. Four treat- ment groups were sensitized by intraperitoneal injection of ovalbumin, locally inhalation of ovalbumin and in- tranasal instillation of ovalbumin, and the fifth group were used as controls. The treatment groups received in- jection of 35 μJ of triamcinotone acetonide, or physiological saline solution into Quchi acupoint, or intramuscular injection of 35 μl of triamcinolone acetonide. Nasal symptom severity in rats with allergic rhinitis was assessed by the number of incidents of sneezing, nasal rubbing and the extent of rhinorrhea. Histomorphological changes of nasal mucosa were observed by microscope. Results： The acupuncture point group had significantly greater im- provement during observation in the sneezing, nasal rubbing and watery, rhinorrhea （P 〈 0.01 compared to thecontrol group）. The intramuscular group and the saline group showed no significant difference from the control group in the nasal symptoms. Histomorphologically, there were significantly interstitial edema, dilation of the small vessels, infiltration of eosinophils and tissue damage under the nasal mucosa of septums in the intramuscu- lar group, the saline group and the model group. The acupoint group and the control group had no pathological lesion as described above. Conclusions ： The injection of triamcinolone acetonide into Quchi acupoint for allergic rhinitis is an effective strategy. The strategy for treating allergic rhinitis may contribute to lessen allergic inflam- matory reaction and block up the allergy, which significantly alleviated the symptom of allergic rhinitis.
