摘　　要：目的了解定点医院结核病防治模式实施效果,为完善医防合作机制提供参考依据。方法 2012年4月招募3省6个地市和县区(山东省德州市夏津县和济宁市兖州市,湖南省岳阳市君山县和长沙市岳麓区,以及甘肃省平凉市崆峒区和张掖市高台县)的卫生行政部门人员、结防机构和定点医院的管理人员和业务人员进行个人深度访谈。结果探索结核病防治新模式,实现优势互补提供更好的诊疗和公共卫生服务是申请实施定点医院模式的主要原因。调查对象认为实施定点医院模式强化了医防合作,提高了结核病诊疗和防控总体工作水平,有助于减少医患纠纷提高患者治疗依从性。调查发现,定点医院模式在防治经费保障、工作职责衔接落实、医院诊疗工作规范性以及患者医疗负担等方面还面临挑战。结论实施定点医院模式取得了积极效果,得到调查对象的普遍认可,应采取措施完善定点医院模式,保障医防合作机制的可持续发展。Objective To assess the implementation of tuberculosis（TB）control model in designated hospitals. Methods In April 2012,managers and operational staff were recruited from health administrative departments,TB dispensaries and designated hospitals in 6counties and cities of 3provinces（Shandong,Hunan,Gansu）for specialized interviews. Results The main reasons for applying TB control model in designated hospital were to explore new control models and to provide better services for TB diagnosis,treatment and prevention,which could enhance the cooperation between hospitals and TB dispensaries,improve the overall quality of TB diagnosis,treatment and prevention,as well as to reduce the medical disputes in order to enhance the treatment compliance of TB patients.However,the implementation of TB control model still faced some challenges in terms of financial guarantee,implementation of collaboration,standardized TB clinical practices and medical burden of TB patients. Conclusions The implementation of TB control model in designated hospitals has yielded some positive results and has been widely accepted.But,it still needs to be further improved.
