摘  要:目的 研究老老年高血压患者2型糖尿病与血清游离脂肪酸之间的关系.方法 纳入中国中医科学院广安门医院2013年11月至2014年7月心内科住院的老老年高血压患者152例,男性68例、女性84例,平均年龄(83.63±3.39)岁.其中伴有2型糖尿病者74例,为A组;非2型糖尿病者78例,为B组.两组患者均检测血脂,比较两组血脂水平的差异.结果 两组患者比较,BMI:A组(28.73±5.49)kg/m2,B组(23.21 ±5.83) kg/m2;空腹血糖:A组(7.01±2.71)mmd/L,B组(5.30±0.85)mmol/L;游离脂肪酸水平:A组(0.62±0.32)mmol/L,B组(0.51 ±0.28)mmol/L;TC:A组(4.16±1.04)mmol/L,B组(3.87±0.93)mmol/L;TG:A组(1.56±0.84)mmol/L,B组(1.31±0.66)mmol/L; HDL-C:A组(1.06±0.25)mmol/L,B组(1.22±0.32)mmol/L;极低密度脂蛋白:A组(0.35±0.29)mmol/L,B组(0.27 ±0.15)mmol/L; APOA1:A组(1.10±0.17)mmol/L,B组(1.17±0.19)mmol/L;脂蛋白:A组(14.64±15.64)mmol/L,B组(21.89±21.46)mmol/L,以上指标两组比较差异有统计学意义(均P<0.05).Spearman相关性分析显示,BMI(r=0.468,P=0.002)、游离脂肪酸(r=0.147,P=0.027)、TC(r=0.147,P=0.028)、HDL-C(r =-0.261,P=0.000)、VLDL(r=0.148,P=0.026)、APOA1 (r=-0.162,P=0.014)、脂蛋白A(r=-0.219,P=0.001)均与2型糖尿病存在相关性.二元Logistic回归分析显示,血清游离脂肪酸水平与老老年高血压合并2型糖尿病关系最为密切.结论 老老年高血压合并2型糖尿病患者脂质代谢紊乱更加严重,应予以足够的重视,而血清游离脂肪酸水平或许可以作为一干预靶点和预测指标.Objective To study the relationship between old elderly hypertensive patients with type 2 diabetes and serum free fatty acids. Methods 152 eases of very elderly hypertensive patients in the department of Cardiology of Guang An Men Hospital attached to China Academy of Traditional Chinese medical science were selected in the period of November 2013 to 2014, 68 cases were males and 84 females, mean age (83.63±3.39) years old, 74 cases were diagnosed type 2 diabetes, we call it group A for short, 78 patients were non-type 2 diabetes, we call it group B for short. The differences of lipid levels between two groups were compared. Results The comparison of the two groups of patients. BMI:A group was (28.73±5.49)kg/m2 and B group was (23.21± 5.83 )kg/m2. Fasting glucose : A group (7.01±2.71 )mmol/L, B group (5.30±0.85)mmol/L. Free fatty acid levels : Group A (0.62±0.32)mmol/L, B group (0.51±0.28)mmol/L. Total cholesterol: A group (4.16±1.04)mmol/L, B group( 3.87±0.93 )mmol/L. Tfiglyceride : A group ( 1.56±0.84 )mmol/L, B group( 1.31+0.66 )mmol/L. High density lipoprotein: A group (1.06±0.25)mmol/L, B group (1.22±0.32)mmo1/L. Very low density lipoprotein:A group (0.35±0.29)mmol/L, B group(0.27±0.15)mmol/L. APOAI: A group(1.10±0.17)mmol/L, B group(1.17±0.19) mmol/L. Lipoprotein. A group ( 14.64 ±15.64)mmol/L, B group (21.89 ±21.46)mmol/L. All P 〈0.05, significant difference. Spearman correlation analysis showed: BMI(r=0.468, P=0.002), free fatty acid(r=0.147, P=0.027), total cholesterol (r=0.147, P=0.028), high density lipoprotein (r=-0.261, P=0.000), VLDL (r=0.148, P= 0.026), APOAl(r=-0.162, P=-0.014), lipoprotein A (r=-0.219, P=0.001) were correlated with type 2 diabetes. The Logistic regression analysis showed that serum levels of free fatty acids and very elderly hypertensive patients with type 2 diabetes was most closely related.
