非小细胞肺癌血液EML4-ALK融合基因检测及临床意义

摘　　要：目的根据肿瘤循环核酸，运用qKT-PCR的方法在血液样本中检测NEML4-ALK融合基因并且简单分析其临床病理特征。方法选取天津胸科医院2012年2月至2013年2月期间，53例非小细胞肺癌患者的血液标本，富集血浆中循环RNA后逆转录成为CDNA，使用qRT-PCR的方法检测EML4-ALK融合基因，并且对阳性表达的样本所对应的组织样本行免疫组化实验明确实验结果。结果在53例血液样本中发现3例EML4-ALK融合基因，其中2例为女性，1例为男性，其差异无统计学意义（P〉0．05）2例患者无吸烟史，1例患者有吸烟史，其差异无统计学意义（P〉0．05）3例病理类型均为腺癌有统计学意义（P〈0．05）此3例EML4-ALK融合基因患者所对应的组织学进行免疫组化实验后均发现EML4-ALK融合蛋白结论：运用qRT—PCR方法能够简便且微创的通过外周血检测EML4-ALK融合基因，这对指导临床靶向治疗有着积极的意义。Objective： According to the circulating nucleic acid, using rt-pcr, from blood samples of Non-small cell lung cancer patients to de- tect EML4 - ALK fusion gene and investigating the clinical and pathological features Methods： 53 cases of non-small cell lung cancer patients blood samples collection of tianjin chest hospital. Amphfication by Polymerase Chain Reacuon （PCR） to circulating RNA in plasma, then detect EML4 - ALK fusion gene. The results of PCR to confirm by immunohistochemistry. Results： Probability ofEML4-ALK gene fusion in a blood sample is 5.66 （3/53）, including 2 cases of fusion gene for E6a/b; A20, 1 case of fusion model for E13; A20. EML-ALK fusion gene is signifi- candy related to lung adenocarcinoma （p=0.049）, but there was no association of EML4-ALK fusion gene with the age（P=0.573） and smoking （P=0.213）. Conclusions： EML4 - ALK fusion gene was detected by PCR in plasma samples of circulating nucleic acids in our experiment whichfound 3 cases EML4 - ALK fusion gene from tianjin chest hospital in 53 patients with non-small cell plasma. 3 cases of samples all from patienu with lung adenocarcinoma.With plasma EML4 - ALK fusion gene detection is a kind of simple and minimally invasive Procedure to assist clinical for more effecdve targeted therapy.
