摘 要:目的:探讨商环包皮切除术治疗男性包皮过长或包茎不同去环时间的临床意义。方法:对从2010年1月至2014年1月在贵州省人民医院接受商环包皮切除术共422例患者进行回顾性研究,比较术后7 d、10 d去环患者的手术时间、手术时疼痛评分、术后24 h内疼痛评分、去环前24 h疼痛评分、去环时疼痛评分、去环后离开疼痛评分、伤口愈合天数、切口裂开发生率。结果:术后7 d、10 d去环患者的手术时间分别为[8~15(9.99±0.15)]min、[5~10(9.97±0.38)]min,两组间差异无统计学意义(P〉0.05);术后7 d、10 d去环患者手术时疼痛评分分别为0~3(0.99±0.83)、0~2(1.00±0.73),两组间差异无统计学意义(P〉0.05);术后7 d、10 d去环患者术后24 h内疼痛评分分别为5~9(6.72±1.03)、5~9(6.76±0.99),两组间差异无统计学意义(P〉0.05);术后7 d、10 d去环患者去环前24小时疼痛评分分别为0~2(1.26±0.71)、0~3(1.12±0.67),两组间差异无统计学意义(P〉0.05);术后7 d、10 d去环患者去环时疼痛评分分别为3~5(3.54±0.47)、3~5(3.57±0.46),两组间差异无统计学意义(P〉0.05);术后7 d、10 d去环患者去环后离开疼痛评分分别为2~4(2.74±0.74)、2~4(2.72±0.71),两组间差异无统计学意义(P〉0.05);术后7 d、10 d去环患者伤口愈合时间分别为[11~17(14.19±1.27)]d、[12~17(13.98±1.29)]d,两组间差异无统计学意义(P〉0.05);术后7 d、10 d去环患者切口裂开发生率分别为4.4%、3.4%,两组间差异无统计学意义(P〉0.05)。结论:商环包皮切除术治疗男性包皮过长7 d去环优于10 d去环。Objective: To evaluate clinical significance of ring-removal at different time points after quotient ring circumcision in males with redundant prepuce or phimosis. Methods: This retrospective study included 422 patients who received quotient ring circumcision at Guizhou Province People's Hospital from January 2010 to January 2014. These patients were grouped into two randomly and received ring-removal after 7 or 10 days respectively. We analyzed the clinical data,including the operation time,VAS pain score during performance within 24 hours after the circumcision,24 hours before ring-removal,during the removal as well as that after removal. The wound healing time and incision dehiscent rate were also observed. Results: The operation time in the two groups were 9. 99 ± 0. 15( 8- 15) min,9. 97 ± 0. 38( 5- 10) min and VAS pain score were 0. 99 ± 0. 83( 0- 3),1. 00 ± 0. 73( 0- 2) during circumcision;6. 72 ± 1. 03( 5- 9),6. 76 ± 0. 99( 5- 9) within 24 hours after the circumcision; 1. 26 ± 0. 71( 0- 2),1. 12 ± 0. 67( 0- 3) 24 hours before ring-removal. The data above were not statistically significant( P〉0. 05). The VAS pain score during the removal were 3. 54 ± 0. 47( range: 3- 5),3. 57 ± 0. 46( 3- 5) and that after removal were 2. 74 ± 0. 74( 2- 4),2. 72 ± 0. 71( 2- 4). Both of them were not statistically significant( P〉0. 05). The wound healing time were 14. 19 ± 1. 27( 11- 17) d,13. 98 ± 1. 29( 12- 17) d and the incision dehiscent rate were 4. 4% and 3. 4%. There was no statistic significance( P〉0. 05). Conclusion: Ring-removal after 7 days was superior to that after 10 days for patients who received quotient ring circumcision.
