摘 要:目的对比分析骨水泥型双极人工股骨头置换术与股骨近端解剖锁定钢板治疗老年股骨转子间粉碎性骨折的临床疗效和术后并发症。方法回顾性选取我科2008年6月-2013年6月因稳定性股骨转子间骨折住院手术治疗的患者共40例,其中行骨水泥型双极人工股骨头置换术20例,行股骨近端解剖锁定钢板(LCP)20例。对比两组患者的手术一般情况、手术前及术后6个月髋关节功能Harris评分和术后弃拐时间、手术并发症发生率。结果骨水泥组患者的手术时间、切口长度、术中出血量、术后引流量、住院天数和下床时间均显著低于LCP组,P〈0.05。两组患者术后6个月的Harris评分均显著高于术前,且骨水泥组术后6个月的Harris评分显著高于LCP组,P〈0.05;骨水泥组的术后弃拐时间显著短于LCP组,P〈0.01。两组患者术后并发症的发生率无显著差异,P〉0.05。结论采取骨水泥型双极人工股骨头置换术治疗老年股骨转子间粉碎性骨折具有手术风险小,患者术后恢复佳的特点,值得临床应用。Objective To compare and analyze the clinical effect of cemented long stem prosthesis with proximal femur locking compression plate for intertrochanteric fractures in elderly patients. Methods Forty patients with stable intertrochanteric hip fractures from Jun. 2008 to Jun. 2013 in our hospital were selected and divided into cemented long stem prosthesis group and locking compression plate( LCP) group,with 20 patients in each group.The general surgery conditions,Harris score before surgery and 6 months after surgery,postoperative weight-bearing time and incidence of surgical complications of two groups were compared. Results The operation time,incision length,blood loss,postoperative drainage,hospital stay and healing time of the cemented long stem prosthesis group were significantly lower than those in the LCP group,P〈0. 05. Harris score in both groups 6 months after surgery were significantly higher than those before treatment,and Harris score of the cemented long stem prosthesis group was significantly higher than that of the LCP group,P〈0. 05. The postoperative weight-bearing time of the cemented long stem prosthesis group was significantly shorter than that of the LCP group,P〈0. 01. The incidence of postoperative complications had no significant difference,P〉0. 05. Conclusion Cemented long stem prosthesis for intertrochanteric fractures for elderly patients has a smaller surgical risk and better postoperative recovery,which is worthy of clinical application.
