摘 要:目的探讨腰椎Modic I型改变的腰椎终板与椎间盘髓核中TNF-α、IL-6、IL-8的相关性。方法收集30例因下腰痛入院手术治疗并行腰椎MRI检查确认为腰椎Modic I型改变患者的术中病灶椎间盘标本,进行免疫组化检测,并观察椎间盘组织病理变化及炎症因子的表达,IL-6、IL-8相对表达水平以相对表达量△Ct或(2^(-△Ct)×10~4)进行分析。结果 Modic I型终板中TNF-o表达水平(9.2±0.7)高于髓核(7.2±0.5),差异有统计学意义(P<0.05);髓核中△Ct IL-6、△Ct IL-8相对表达量(6.34±1.82、10.62±1.88)高于终板(4.07±1.86、5.92±1.31),差异均有统计学意义(均P<0.05)。结论 Modic I型终板中TNF-α、IL-6、IL-8的表达水平明显高于髓核,其引起下腰痛的原因主要为终板的炎症改变。Objective To investigate the expression of TNF-α, IL-6 and IL-8 in lumbar vertebral nucleus pulposus and end plate tissue in patients with Modic type I changes. Methods Thirty patients with low back pain and MRI-confirmed lumbar end-plate Modic type I changes underwent surgical treatment. The disc specimens were collected in the surgery and examined with Immunohistochemistry for expression of TNF- α, IL-6 and IL-8. The association of pathological changes of intervertebral disc tissue with the expression of TNF- α, IL-6, IL-8 and the relative expression of Ct or(2^-△Ct × 10^4) was analyzed. Results The expression level of TNF-α in lumbar end plates was higher than that in nucleus pulposus (9.2 ± 0.7, 7.2 ± 0.5, P〈0.05); while the expressions of IL-6 and IL-8 in nucleus was higher than those in end-plate (6.34 ± 1.82, 4.07 ±1.86 and 10.62± 1.88, 5.92± 1.31, respectively, P〈0.05). Conclusion The expression of inflammatory factors in lumbar endplate were above that of nucleus. The low back pain was caused by the inflammation changes in lumbar end plate.
