丹参川芎嗪联合厄贝沙坦氢氯噻嗪片治疗难治性高血压的效果及保肾作用

摘　　要：目的研究丹参川芎嗪联合厄贝沙坦氢氯噻嗪片治疗难治性高血压的临床效果及保肾作用。方法选择2013年10月~2014年11月在江苏省沭阳县中医院接受治疗的难治性高血压患者68例,根据随机数字法分成观察组及对照组,各34例,对照组单纯给予厄贝沙坦氢氯噻嗪片治疗,观察组在此基础上另给予丹参川芎嗪治疗,治疗2个疗程,对比两组患者临床疗效及不良反应发生率,并对两组治疗前后的血压变化和肾功能相关指标进行比较。结果观察组总有效率为94.12%,显著高于对照组的73.53%,差异有统计学意义（P〈0.05）;观察组治疗后的收缩压及舒张压水平均显著低于对照组,差异均有高度统计学意义（均P〈0.01）;观察组治疗后胱抑素C、尿微量白蛋白、血肌酐及尿素氮水平均显著低于对照组,差异均有高度统计学意义（均P〈0.01）。观察组总不良反应发生率为17.65%,与对照组的29.41%比较,差异无统计学意义（P〉0.05）。结论丹参川芎嗪及厄贝沙坦氢氯噻嗪联合治疗难治性高血压患者,可获得较好的疗效,且具有明显的保肾作用,效果明显,值得推荐。Objective To study the clinical curative effect and the renal protective effects of Salvia miltiorrhiza Ligustrazine combined with Irbesartan and Hydrochlorothiazide Tablets in treatment of refractory hypertension. Methods 68 cases of patients with resistant hypertension who were treated in Shuyang Hospital of Traditional Chinese Medicine from October 2013 to November 2014 were selected, and they were randomly divided into observation group and control group, with 34 cases in each group according to the figures, the control group were given simply Irbesartan and Hydrochlorothiazide Tablets, the observation group were given Salvia miltiorrhiza Ligustrazine based on it, after 2courses of treatment, the clinical curative effect and adverse reactions, and the blood pressure changes, the related indexes of renal function before and after treatment were compared between two groups. Results The total effective rate of observation group was 94.12%, significantly higher than 73.53% of the control group, the difference was statistically significant（P〉0.05）. After treatment, the SBP and DBP level of the observation group were significantly lower than the control group, the differences were statistically significant（P〈0.01）. The levels of Cys C, urine m ALB, Scr and BUN after treatment in the observation group were significantly lower than those in the control group, the differences were statistically significant（P〈0.01）. The total adverse reaction incidence rate of the observation group was 17.65%,compared with 29.41% in the control group, it had no significant difference（P〉0.05）. Conclusion Salvia miltiorrhiza Ligustrazine combined with Irbesartan Hydrochlorothiazide to treat patients with resistant hypertension, can obtain good effect, and has renal protective effects, obvious effect is obvious, which is worthy of recommendation.
