Foley导尿管在颅底骨折伴失血性休克中的应用

摘　　要：目的探讨Foley导尿管在颅底骨折伴失血性休克中的应用。方法将84例急诊收治的颅底骨折伴失血性休克患者随机分为对照组和Foley组,各42例。2组患者入院后均建立静脉通道、补液扩容等生命支持治疗。Foley组通过Foley导尿管压迫止血。对照组通过后鼻道填塞压迫止血。分别对2组止血时间、止血效果、感染机会、抢救成功率进行评估。结果 Foley组较对照组在止血时间上优势明显,感染机会、抢救成功率与对照组比较差异有统计学意义(P<0.05),止血效果差异无统计学意义(P>0.05)。结论 Foley导尿管在颅底骨折伴失血性休克抢救中,操作简单、疗效确切,为下一步治疗赢得宝贵时间,值得进一步实施和推广。Objective To discuss the application of Foley catheter in skull fracture with hemorrhagic shock. Methods Totally 84 emergency patients diagnosed as skull fracture with hemorrhagic shock in this hospital were randomly divided into the control group（ n = 42） and Foley group（ n = 42）. 42 cases in Foley group received the life- sustaining treatment such as the establishment of vein access and rehydration and expansion and the hemostasis by compression through Foley catheter; 42 cases in the control group received the life- sustaining treatment such as the establishment of vein access and rehydration and expansion and the hemostasis by compression through the posterior packing. The hemostasis time,hemostasis effect,infection chance and successful rescue rate for patients with skull fracture and hemorrhagic shock were evaluated. Results Foley group performed significantly better in the hemostasis time,infection chance and successful rescue rate than that of the control one,with the statistical difference（ P〈0. 05）. But there was no significant difference in the hemostasis effect between two groups（ P〉0. 05）. Conclusion Due to the simple operation and outstanding curative effect in the rescue of skull fracture with hemorrhagic shock,Foley catheter gain the valuable time for the next step of treatment and thus it is worthy to be further implemented and popularized.
