异丙酚联合外旋法、手牵足蹬法治疗急性肩关节前脱位的临床疗效

摘　　要：目的:观察急性肩关节前脱位患者采用外旋法、手牵足蹬法治疗时应用异丙酚的临床疗效。方法:选取2012年6月—2014年6月期间符合纳入标准的66例急性肩关节前脱位患者进行研究。按照随机数字表法分组为对照组和观察组。对照组:采用外旋法、手牵足蹬法治疗,但不采用任何麻醉。观察组:采用异丙酚麻醉再采用外旋法、手牵足蹬法治疗。观察两组患者复位成功率和复位操作持续时间及疼痛程度、并发症、功能恢复情况。结果:观察组复位成功率为87.88%(29/33)与对照组72.73%(24/33)比较(P<0.05)。观察组复位操作持续时间和疼痛评分分别为(2.07±0.95)min、(2.28±1.49)分与对照组(3.68±1.19)min、(4.75±2.08)分比较(P<0.05)。观察组UCLA评分为(33.17±1.80)分与对照组(32.96±1.91)分比较(P>0.05)。结论:急性肩关节前脱位患者采用外旋法、手牵足蹬法治疗时应用异丙酚可减轻患者疼痛,促进治疗顺利进行,节约治疗时间,提高复位成功率;但两组患者功能恢复效果相当。Objective： To observe the clinical efficacy of propofol combined external rotation and hand holding and foot stepping method for patients with acute anterior dislocation of shoulder joint. Methods： During June 2012-June 2014,66 cases of patients with acute anterior dislocation of shoulder joint were studied. According to the random number table method,they were grouped into control group and observation group. Control group： external rotation and hand holding and foot stepping method without any anesthesia. Observation group： propofol anesthesia and external rotation and hand holding and foot stepping method. We observed two groups＇ reset success rate and reset operation duration and degree of pain,complications and functional recovery. Results： Observation group＇s reset success rate was 87. 88%（29 /33） and that of the control group was 72. 73%（24 /33）（P〈0. 05）. The observation group＇s reset operation duration and pain score were（2. 07 ± 0. 95） min and（2. 28 ± 1. 49） respectively and the control group＇s were（3. 68 ± 1. 19） min and（4. 75 ± 2. 08） points（P〈0. 05）. The observation group＇ s UCLA score was（33. 17± 1. 80） and it was（32. 96 ± 1. 91） in the control group（P〉0. 05）. Conclusion： Propofol combined with external rotation and hand holding and foot stepping method for patients with acute anterior dislocation of shoulder joint can alleviate patients＇ pain,promote the progress of the treatment to save time and improve the success rate reset. But the effect of two groups＇ function recovery was the same.
