经导管左心耳封堵术预防心房颤动相关脑卒中临床研究进展

摘　　要：心房颤动是临床最常见的心律失常之一,是临床上导致缺血性卒中的重要原因,具有高致残率及致死率。其栓子主要来源左房,而60%的风湿性心脏病及90%的非风湿性疾病心房颤动患者血栓来源于左心耳。口服抗凝药治疗是目前心房颤动患者的常规治疗方案,但因其较高的出血风险、合并抗凝禁忌证及服药依从性等问题,治疗率及达标率并不理想。近年来,经导管左心耳封堵术已在部分临床实践中证实了其预防心房颤动相关脑卒中方面良好的安全性和效果,现就其临床研究进展进行综述。Atrial fibrillation is one of the most common arrhythmias,and it is an important reason which leads to ischemic stroke and there’s high disability and mortality with atrial fibrillation patients. The thrombus in patients with atrial fibrillation mainly originates from left atrium,and thrombi in patients with rheumatic valve atrial fibrillation originate from the left atrial appendage is 60% and more than 90% of thrombi is found in the left atrial appendage in patients with nonrheumatic valve atrial fibrillation. Oral anticoagulation therapy is the conventional treatment in patients with atrial fibrillation recently,but there are many problems such as high bleeding risk,anticoagulation contraindication and poor compliance. The rate of treatment and reaching the standard is not ideal. Recently,the transcatheter closure method is confirmed favorable safety and effectiveness in part of clinical practice. This paper reviews the clinical advances in the left atrial appendage occlusion methods.
