益气通腑逐瘀方灌肠疗法对脓毒症患者血液流变性的影响

摘　　要：目的观察益气通腑逐瘀方灌肠疗法对脓毒症患者血液流变学的影响。方法 80例脓毒症患者,随机分治疗组和对照组各40例。对照组给予常规治疗,治疗组在对照组基础上加用益气通腑逐瘀方中药灌肠治疗。两组分别在治疗前及治疗后第5日、第10日,检测全血高切黏度、全血低切黏度、血浆黏度、红细胞刚性指数、红细胞聚集指数、血沉方程K值、红细胞变形指数。结果治疗10 d后,治疗组治疗组总有效率77.50%高于对照组的52.50%(P<0.05)。两组治疗10 d后中医证候评分、APACHEⅡ评分均较治疗前明显下降(均P<0.05),且治疗组评分改善优于对照组(均P<0.05)。治疗后两组血液流变学指标全血高切黏度、全血低切黏度、血浆黏度、红细胞刚性指数、红细胞聚集指数、血沉方程K值、红细胞变形指数均较本组治疗前改善(均P<0.05),且治疗组改善水平显著优于对照组(均P<0.05)。结论益气通腑逐瘀方灌肠疗法能够促进脓毒症患者血液循环、改善血液黏度、防止血液溶解、促进疾病恢复。Objective： To observe the effect of Yiqi Tongfu Zhuyu Tang enema therapy on blood rheology in patients with sepsis.Methods： 80 cases of sepsis were randomly divided into two groups including the treatment group（40 cases） and the control group（40 cases）.Both group were given conventional treatment.The treatment group was combined Yiqi Tongfu Zhuyu Tang enema treatment.There were some indices observed in these groups such as the whole blood high shear viscosity,the whole blood low shear viscosity,the plasma viscosity,the erythrocyte rigidity index,the red blood cell aggregation index,the blood sedimentation equation K value,the Erythrocyte deformation index taken to measure respectively before treatment and in 5 and 10 days after the treatment.Results： After treatment,the total effective rate of treatment group was significantly higher than that of the control group.The TCM symptom score and the APACHE Ⅱ score of the treatment group were significantly decreased than those of the control group.These indices were also significantly lower than those of the control group including the whole blood high shear viscosity（200/s）,the whole blood low shear viscosity,the plasma viscosity,the erythrocyte rigidity index,the red blood cell aggregation index,the blood sedimentation equation K value.However,the Erythrocyte deformation index was significantly higher than that of the control group.Conclusion：Yiqi Tongfu Zhuyu Tang enema therapy can promote the blood circulation,improve blood viscosity,prevent blood dissolved and promote disease recovery.
