糖皮质激素对结核性胸膜炎疗效的Meta分析

摘　　要：目的评价糖皮质激素治疗结核性胸膜炎的疗效。方法计算机检索关于糖皮质激素对结核性胸膜炎疗效的随机对照研究,同时追索纳入文献的参考文献。在严格质量评价的基础上,应用Rev Man5.1软件进行Meta分析。结果纳入的文献共9篇,包含665个研究对象。Meta分析结果显示,糖皮质激素治疗结核性胸膜炎在治疗效果上与对照组比较差异有统计学意义(P<0.05);糖皮质激素治疗结核性胸膜炎发生胸膜增厚粘连比对照组低,差异有统计学意义(P<0.05);糖皮质激素治疗结核性胸膜炎不良反应的发生要高于对照组,差异有统计学意义(P<0.05)。结论早期使用激素可以迅速的缓解中毒症状,减少和防止胸膜增厚粘连,但是不良反应的发生率将增高。Objective To evaluate the efficacy of glucocorticoid in the treatment of tuberculosis pleuritis. Methods Researches of randomized controlled trials （RCTs） on the curative effect of glucocorticoid on tuberculous pleuritis as well as relevant bibliographies were searched on the Internet. On the basis of strict quality evaluation, Rev-Man 5.1 software was used for meta analysis. Results A total of 9 researches with 665 research objects were included. The results of meta analysis showed that there was statistically significant difference of the curative effect between the treatment group and control group （P〈0.05）. The incidence of pleural pleural thickening and adhesion in treatment group was lower than that in the control group, and the difference was statistically significant （P〈0.05）. The occurrence of adverse reactions in treatment group was higher than that in the control group, and the difference was statistically sig-nificant （P〈0.05）. Conclusion Early application of glucocorticoid can quickly alleviate symptoms of poisoning, re-duce and prevent pleural thickening and adhesion. However, the incidence rate of adverse reaction will be increased.
