自制复方利多卡因药卷用于痔上黏膜环切术后早期镇痛

摘　　要：目的探讨自制复方利多卡因药卷在吻合器痔上黏膜环切钉合术后早期镇痛中的应用效果。方法将行吻合器痔上黏膜环切钉合术的186例患者随机分成利多卡因组(91例)和对照组(95例)。利多卡因组于术后即采用自制复方利多卡因药卷填塞于肛管,并于术后第1天早晨换药时追加1次;对照组术后直接用明胶海绵填塞于肛管。比较两组术后肛门伤口24h内疼痛程度、肛门坠胀感、第1次排便时疼痛感及镇痛药使用率。结果利多卡因组术后24h内肛门伤口疼痛、肛门坠胀感及第1次排便时疼痛得分显著低于对照组,术后24h内镇痛药使用率显著低于对照组(P<0.05,P<0.01)。结论自制复方利多卡因药卷用于吻合器痔上黏膜环切钉合术后早期镇痛,效果较好,可减轻患者痛苦,减少镇痛药的使用率,促进患者术后康复。Objective To explore the efficacy of homemade compound lidocaine cartridge in postoperative analgesia after procedure for prolapse and hemorrhoid（PPH）.Methods A total of 186 patients receiving PPH were randomly divided into two groups.The lidocaine group（91cases）received a homemade compound lidocaine cartridge to pack the anal canal after the surgery and another cartridge was added on the second postoperative day.The control group（95cases）were given gelatin sponge to pack the anal canal.Results The scores of severity of pain within 24 hours postoperatively,bulge in anal area,and pain severity during the first-time defecation,as well as the rate of analgesics use,in the lidocaine group were significantly lower than the control group（P〈0.05,P〈0.01）.Conclusion The homemade compound lidocaine cartridge used in PPH can relieve pain,reduce analgesic use,and promote postoperative recovery.
