摘　　要：目的观察通窍调神法对血管性痴呆(VD)患者认知功能及事件相关电位P300的影响。方法将80例VD患者随机分为2组,均接受基础治疗,治疗组40例采用针刺配合中药通窍活血汤口服,对照组40例采用石杉碱甲口服治疗,应用简易智力状态检查量表(MMSE)和日常生活能力量表(ADL:改良Barthel指数法)评分判断认知功能状态及日常生活能力,并检测治疗前后事件相关电位P300变化。结果 2组治疗后事件相关电位P300潜伏期均较治疗前缩短,波幅升高(均P<0.01),但2组治疗后比较差异均无统计学意义(P>0.05)。2组治疗后MMSE、ADL评分与本组治疗前比较差异有统计学意义(P<0.01);2组治疗后MMSE评分比较差异无统计学意义(P>0.05),治疗后ADL评分治疗组优于对照组(P<0.01);治疗组总有效率75%,对照组总有效率70%,2组比较差异无统计学意义(P>0.05)。结论通窍调神法治疗VD疗效确切,能改善患者认知功能,提高患者的日常生活能力。Objective To investigate the effect of orifice - freeing and regulating the vitality method on the cognitive function and event related potential P300 in patients with vasCUlar dementia. Methods 80 patients with vascular dementia were randomly divided into two groups. Patients in two group received essential therapy. At same time 40 cases in treatment group received acupuncture combined with tlae oral Chinese medicine treatment. 40 cases in control group were treated by oral huperzine A. State of cognitive function and daily life ability in two groups were estimated by MMSE and ADL, the changes of event related potential P300 were detected before and after treatment. Results Event related potential P300 latency period after treatment were shortened and amplitude was increased in two groups （ P 〈0.01 ）. There was no difference between two groups （P 〉0.05）. There was significant difference between before and after treatment in two groups on MMSE and ADL （P 〈 0.01 ）. But there was no difference be- tween two groups on MMSE （ P 〉 0.05 ）. ADL in treatment group was superior to that in control after treatment （ P 〈 0.01 ）. There was no difference between two groups on the total effective rate （ P 〉 0.05 ）. Conclusion Ori- fice -freeing and regulating the vitality method has definite effect on vascular dementia, and can improve the ability of the patient＇s daily life, event related potential P300 and help in the diagnosis of vascular dementia symptoms.
