中西医结合治疗急性前壁心肌梗死合并急性左心力衰竭冠脉支架植入术后30例疗效观察

摘　　要：目的观察中西医结合治疗急性前壁心肌梗死合并急性左心力衰竭(以下简称心衰)患者冠脉支架植入术后的临床疗效。方法将急性前壁心肌梗死合并急性左心衰并完成冠脉支架植入术患者60例随机分为2组。对照组30例予多巴酚丁胺静脉持续泵入,治疗组30例在对照组基础上加用中药方口服,共用药5 d。观察2组患者临床疗效、中医临床疗效,用药前后分别进行心肌血清学标志物检测及超声心动图检查。结果 2组临床疗效比较,治疗组总有效率93.3%,优于对照组(80.0%,P<0.05)。中医临床疗效比较,治疗组总有效率93.3%,优于对照组(73.3%,P<0.05)。2组治疗后心肌血清学标志物及超声心动图各项指标较治疗前均改善(P<0.05),且治疗组改善更明显(P<0.05)。结论中西医结合治疗急性前壁心肌梗死合并急性左心衰冠脉支架植入术后患者疗效确切。Objective To observe the curative effect of combined therapy of Chinese and Western medicine on the treatment of patients with the anterior wall acute myocardial infarction with acute left heart failure intracoronary stent implantation. Methods 30 cases of the anterior wall acute myocardial infarction with acute left heart failure in- tracoronary stent implantation were randomly divided into two groups. 15 cases in control group were treated by Dobu- tamine venous continuous pumping. 15 cases in treatment group were treated by oral administration of Chinese medi- cine prescription on the basis of control group treatment. The course was five days in two groups. The clinical effect and traditional Chinese medicine clinical effect were observed. Respectively before and after medication myocardial serological markers detection and echocardiography were done. Results Aspect of two groups of clinical curative effect comparison, the total effective rate of clinical effect in treatment group （93.3%） was higher than that in con- trol group （ 80.0%, P 〈 0.05 ）. Aspect of two groups of traditional Chinese medicine clinical curative effect compar- ison, the total effective rate of clinical effect in treatment group （93.3%） was higher than that in control group （73.3%, P 〈0.05 ）. The indexes of myocardial serological markers detection and echocardiography after treatment were improved as compared with those before treatment （ P 〈 0.05 ）. The improvement in treatment group was more obvious than that in control group （ P 〈 0.05 ）. Conclusion Combined therapy of Chinese and Western medicine has definite effect on the treatment of patients with the anterior wall acute myocardial infarction with acute left heart failure intracoronary stent implantation.
