731例尖锐湿疣患者人乳头瘤状病毒基因分型及其感染部位的相关性研究

摘  要:目的:探讨尖锐湿疣(CA)患者人乳头瘤状病毒(HPV)基因分型及其与感染部位的相关性。方法:对731例CA患者病损组织或宫颈脱落细胞提取HPVDNA,应用基因芯片技术检测其基因型别,并分析患者临床感染部位资料。结果:731例尖锐湿疣患者的组织或分泌物标本,HPV总阳性率为89.19%。HPV亚型:379例低危型构成中,HPV6、11、42、43、44型依次为49.60%、47.76%、1.85%、0.53%、0.26%。273例高危亚型构成中, HPV16、18、31、33、35、45、51、58型依次为32.60%、35.53%、8.42%、4.39%、7.32%、6.59%、2.94%、2.20%。HPV多重感染:652例阳性标本中,单一基因型感染占70.40%,二重感染占18.10%,三重感染占5.85%,四重感染占3.83%,五重感染及以上占1.84%。HPV感染部位:感染冠状沟的HPV亚型依次是HPV6、11、18、16型;感染尿道口的HPV亚型依次是HPV11、6、16、18型;感染肛周部位的HPV亚型依次为HPV11、6、16、18型;感染外阴部位的HPV亚型依次为HPV6、11、16、18型;感染宫颈部位的HPV亚型依次为HPV16、18、6、11型。结论:尖锐湿疣患者人乳头瘤病毒感染以单一6、11型为主,16、18高危亚型在宫颈处感染率高于其它处,并且表现为多重感染;HPV基因亚型与感染部位具有一定的相关性。Objective:To investigate the effects of condyloma acuminatum (CA) in patients with human papil oma virus (HPV) genotyping and its correlation with the infection. Methods:Of 731 patients with CA lesions tissues or cervical exfoliated cel s extracted HPVDNA, application of gene chip technology to detect the genotype, and analyze the clinical infection site information.Results:731 cases of patients with condyloma acuminatum tissues or secretion specimens, HPV total positive rate was 89.19%. HPV subtypes:of 379 cases of low-risk type composition, HPV6 type, 11, 42, 43, 44, in turn, 49.60%, 47.76%, 1.85%, 47.76%and 0.26%. The constituents of the 273 cases of high-risk subtype, HPV16, 18, 31, 33, 35, 45, 51, 58, in turn, 32.60%, 35.53%, 8.42%, 4.39%, 7.32%, 6.59%, 2.94%, 6.59%. HPV infection of multiple:of 652 cases of positive specimens, a single genotype infection accounted for 70.40%, double infection (18.10%), triple infection (5.85%), four heavy infection accounted for 3.83%, more than five heavy infection, and accounted for 1.84%. HPV infection site:infection coronary ditch of HPV subtypes were HPV6, 11, 18, 16 type;Urethral infection of HPV subtypes HPV11, 6, 16, 18 type;Site crissum infection of HPV subtypes of HPV11 type, 6, 16, 18;Site genital infection of HPV subtypes of HPV6, 11, 16, 18;Site cervical infection of HPV subtypes of HPV16, 18, 6, 11 type. Conclusions:pointed condyloma patients with human papil oma virus infection is given priority to with a single type 6, 11, 16, 18 infection rate is higher than other area to the high-risk subtypes in the cervix, and characterized by multiple infection;HPV gene subtype and the infection has certain correlation.
