悬吊式免气腹经脐单孔腹腔镜阑尾切除术的临床分析

摘　　要：目的：探讨悬吊式免气腹经脐单孔腹腔镜阑尾切除术的可行性及临床应用价值。方法：回顾分析2013年8月至2014年5月为20例患者行悬吊式免气腹经脐单孔腹腔镜阑尾切除术的临床资料,术中使用骨科牵引弓、克氏针自制腹壁悬吊装置及普通腹腔镜操作器械完成手术。结果：手术均获成功,无中转常规腹腔镜手术及开腹手术,手术过程顺利,手术时间40~70 min,平均（54.0±1.7）min;无腹腔、脐部切口感染,平均住院（6.0±1.2）d,患者均顺利出院。结论：悬吊式免气腹经脐单孔腹腔镜阑尾切除术具有微创及美容的优势,使用普通腹腔镜操作器械也可完成手术,具有临床推广价值。Objective：To evaluate the feasibility and the clinical application value of suspensory gasless transumbilical singleport laparoscopic appendectomy（ LA）. Methods：From Aug. 2013 to May 2014,the clinical data of 20 patients who underwent suspensory gasless transumbilical single-port LA were retrospectively analyzed. In operation,orthopedic surgery traction bow,Kirschner wire and ordinary laparoscopic operation instruments were used to complete the operation. Results：The operations were successfully and uneventfully completed without conversion to routine laparoscopic surgery or open surgery in all of the cases. The mean operation time was（54. 0 ± 1. 7） min（ranged from 40. 0 to 70. 0 min）. There were no infection of the umbilical wound and abdominal cavity. The average hospital stay was（6. 0 ± 1. 2） d. All patients were discharged from the hospital. Conclusions：Suspensory gasless transumbilical singleport LA has the advantages of fewer trauma and better cosmetic result. The operation can be finished with ordinary laparoscopic operation instruments. This surgery has the clinical application value and should be spreaded.
