摘 要:目的:探讨慢性酒精中毒性脑病(CAE)患者经颅多普勒改变及临床应用价值。方法:对33例符合诊断标准的CAE患者和39例正常健康者进行经颅多普勒(TCD)检测,并将结果进行对比分析。结果:CAE患者和正常健康者在双侧大脑中动脉的平均血流速度(Vm)上的差异无统计学意义(P>0.05)。但CAE患者的脑血管搏动指数(PI)及血流频谱形态异常率比正常健康者高。两组在PI值及频谱形态的比较上差异具有统计学意义(P<0.05)。在CAE患者血流频谱形态异常率中,饮酒时间长及饮酒量大的患者所占的比率高,其差异具有统计学意义(P<0.05)。结论:TCD能够发现CAE患者的脑血流动力学改变情况,从而为CAE患者的继发脑血管损伤评估及治疗提供有价值的依据和参考。Objective: To investigate the transcranial doppler (TCD) change and value in patients chronic alcoholism encephalopathy(CAE). Methods: 33 patients with CAE and 39 normal subjects received tests of TCD and all the results were compared and analyzed. Results: It is no statistically significant differences of the mean velocity(Vm) in bilateral middle cerebral artery between CAE patients and normal subjects.The abnormal rates of the pulsation index (PI)and pattern of waveform in patients with CAE is higher than that of normal subjects (P 〉 0.05 ) . Both the groups were statistically significant difference between the PI and pattern of waveform(P〈 0.05). The abnormal rate of waveform pattern with CAE patients was high in the case with large alcohol consumption and in those with the time length, the statistical difference was significant (P 〈 0.05). Conclusion: TCD can find the change of cerebral hemodynamic in patients with CAE and provide the valuable basis and reference for clinical evaluation and treatment after secondary injuries of cerebral vessel in patients with CAE
