Graves'病患者^(131)I治疗前后血清TNF-α、IL-8及LEP测定的临床意义

摘　　要：目的探讨Graves'病患者131I治疗前后血清TNF-α、IL-8、瘦素水平变化的临床意义。方法用放射免疫分析法对初诊32例Graves'病女性患者131I治疗前后及30例健康查体者血清TNF-α、IL-8、leptin含量进行测定。结果 32例Graves'病患者治疗前血清瘦素含量较正常对照组低(P<0.01);TNF-α、IL-8含量显著高于正常对照组(P<0.01)。经131I治疗FT3、FT4、s TSH恢复正常后,血清瘦素含量较治疗前明显升高(P<0.01),与对照组比较无显著性差异(P>0.05);TNF-α、IL-8较甲状腺机能亢进组均明显降低(P<0.01)但仍高于正常对照组(P<0.05)。结论血清瘦素的变化可作为判断Graves'病患者病情和疗效的指标之一;131I治疗前后TNF-α、IL-8的变化可反映Graves'病的免疫动态,可能与发病机制、治疗效果和预后有关。Obiective To investigate the clinical significance of TNF- α, IL-8 and LEP changes in serum of female Graves disease patients before and after ^131I treatment. Methods TNF- α, IL-8, LEP level of 32 Graves＂ disease patients were separately examined by RIA before and after ^131I treatment, 30 normal women as control. Results The serum LEP level of 32 Graves＂ disease patients before treatment was obviously lower than that in the normal control group（P〈0.01）; Levels of TNF- α, IL-8 were significantly higher than the normal control group（P〈0.01）. After ^131I treatment FT3, FT4 sTSH were back to normal, the serum level of LEP was significantly higher than that before treatment （P〈0.01）, compared with the control group there was no significant difference （P〉0.05）. Levels of TNF- α, IL-8 dropped significantly （P〈 0.01）, but were still higher than the normal control group（P〈0.05）. Conclusion The fluctuation of serum LEP level could be used as one of the indicators which can evaluate the condition and effect of patients. The fluctuation of TNF- α, IL-8 level can reflect the dynamics immune of Graves disease before and after ^131I treatment. They are related to the pathogenesis, treatment and prognosis.
