表观扩散系数鉴别肝细胞癌及血管瘤价值Meta分析

摘　　要：目的探讨扩散加权成像(DWI)中不同范围b值的平均表观扩散系数(ADC)值在鉴别肝细胞癌及肝血管瘤的价值。方法电子检索Pubmed、中国生物医学文献数据库(CBMdisc)及中国学术期刊网全文数据库(CNKI)、万方数据库1990年1月至2013年11月公开发表的中、英文文献,按照诊断试验的纳入标准筛选关于MRI DWI定量鉴别肝细胞癌及肝血管瘤诊断价值的研究,提取各研究平均ADC值进行汇总分析,计算标准化均数差(SMD)值及95%CI。结果当b值≤400 s/mm2、500 s/mm2≤b值≤700 s/mm2、800 s/mm2≤b值≤2000 s/mm2时,肝细胞癌及血管瘤的平均ADC值的SMD(95%CI)分别为-0.85(-2.46~0.76),-0.92(-1.96^-0.78),-0.74(-1.83~0.35),当500 s/mm2≤b值≤700 s/mm2时,肝细胞癌及血管瘤ADC值差异均有统计学意义。结论当500 s/mm2≤b值≤700 s/mm2时,ADC值在鉴别肝细胞癌及血管瘤方面可能更具有价值。Objective To assess the value of mean apparent diffusion coefficient（ ADC） values depending on b value ranges of diffusion weighted imaging（ DWI） in differentiating hepatic cancer and hepatic hemangioma. Methods By searching the published literatures in Chinese and English from Jan. 1990 to Nov. 2013 in Pubmed,CBMdisc,CNKI and WANFANG,studies on the differentiation of hepatic cancer and hepatic hemangioma by MRI DWI were selected according to the including criteria of diagnostic tests,standardized mean differences（ SMD） of mean ADC values and 95% confidence intervals（ CI） were analyzed and calculated. Results b value ranged from 0 to 400 s/mm2 with a SMD（ 95% CI） of- 0. 85（- 2. 46,0. 76）,500 to 700 s/mm2 with that of- 0. 92（- 1. 96,- 0. 78）,and 800 to 2000 s/mm^2 with that of- 0. 74（- 1. 83,0. 35）. There were no statistically significant differences of mean ADC values between hepatic cancer and hepatic hemangioma when b value ranged from 0 to 400 s/mm^2 and 800 to 2000 s/mm^2,and there were statistically significant differences when b value ranged from 500 to 700 s/mm^2. Conclusion ADC values in diffusion-weighted MRI may have more potential value in differentiating hepatic cancer and hepatic hemangioma when b value ranged from 500 to 700 s/mm^2.
