结直肠癌手术疗效及其影响因素分析

摘　　要：目的探究并分析结直肠癌手术疗效及其影响因素。方法选择2009年1月至2013年1月解放军第十五医院、乌苏市人民医院和澳洋医院普外科就诊的198例结直肠癌患者为研究对象,分析结直肠癌手术疗效及其影响因素。结果手术近期疗效差者占18.7%（37/198）。多因素Logistic回归分析示,年龄〉80岁（β=1.58,OR=4.88,95%CI 3.50~6.79,P=0.000）、淋巴结远处转移（β=2.59,OR=13.29,95%CI 5.22~33.79,P=0.000）、肿瘤分化程度低（β=2.74,OR=15.45,95%CI 8.44~28.27,P=0.000）、营养不良（β=2.00,OR=7.41,95%CI 3.47~15.81,P=0.000）和糖化血红蛋白水平高（β=2.31,OR=10.03,95%CI 6.13~16.45,P=0.000）是影响结直肠癌患者手术疗效的危险因素。结论结直肠癌患者手术疗效总体满意,手术疗效的优劣受多方面因素影响。Objective To investigate the efficacy and influence factors of surgical treatment for colorectal cancer. Methods The clinical data of 198 patients with colorectal cancer treated in the 15 thHospital of Chinese PLA,Wusu People＇s Hospital and Aoyoung Hospital from Jan. 2009 to Jan. 2013 were analyzed retrospectively. And the efficacy and influence factors of surgical treatment for colorectal cancer were analyzed.Results Cases with poor short-term curative effect accounted for 18. 7%（ 37 /198）. Results of multivariable Logistic regression analysis showed that age 80 years old（ β = 1. 58,OR = 4. 88,95% CI 3. 50-6. 79,P = 0. 000）,distant metastasis of lymph node（ β = 2. 59,OR = 13. 29,95% CI 5. 22-33. 79,P = 0. 000）,low tumor differentiation degree（ β = 2. 74,OR = 15. 45,95% CI 8. 44-28. 27,P = 0. 000）,malnutrition（ β =2. 00,OR = 7. 41,95% CI 3. 47-15. 81,P = 0. 000） and high glycosylated hemoglobin level（ β = 2. 31,OR =10. 03,95% CI 6. 13-16. 45,P = 0. 000） were the risk factors of the curative effect of surgical treatment for colorectal cancer. Conclusion The overall curative effect of surgical treatment for colorectal cancer is satisfactory,the disadvantages of the surgery are impacted by various factors.
