我国循证护理干预对高血压病患者健康状况影响的Meta分析

摘　　要：目的评价循证护理干预对高血压病患者健康状况的影响。方法计算机检索PubMed、EMbase、CMB、The Cochrane Library(2013年第11期)、CNKI、VIP和WanFang Data等数据库,并辅以手工检索和其他检索,收集中文发表的循证护理干预对高血压病患者健康状况影响的随机或半随机对照试验,检索时限均为从建库至2013年11月。由2位评价者按照纳入与排除标准独立筛选文献并提取资料,经质量评估后,使用RevMan 5.1软件对纳入文献进行Meta分析。结果依照纳入和排除标准,共纳入15项研究,包括1711例高血压病患者。纳入研究的质量均较低。Meta分析结果显示,与常规护理相比,循证护理干预可提高患者的服药依从性(RR=1.90),降低收缩压(MD=-8.16),提高患者的血压控制率(RR=1.53)及健康知识知晓率(RR=1.94),差异均有统计学意义(均P<0.05)。结论循证护理干预可以提高高血压病患者的服药依从性及健康知识水平,对控制和稳定血压有积极意义;但由于纳入分析的研究质量较低,上述结论尚有待设计更为严谨的研究进一步验证。Objective To evaluate the effect of evidence-based nursing intervention on the health status of patients with hypertension.Methods Search was conducted in the following databases：PubMed,EMbase,The Cochrane Library,CNKI,VIP,WanFang and supplemented with hand-search and other searches during the period from the database established time to November 2013 to collect the Chinese version of randomized or semi-randomized comparison study of effect of evidence-based nursing intervention on the health status of patients with hypertension.Two participator independently screened the literature based on standards to refined the content and evaluate the quality,and Meta-analysis was conducted by RevMan5.1software to assessed the included studies.Results A total of 15 studies involving 1711 patients were included.The quality of included trials was low.Meta-analyses showed that evidence-based nursing intervention improved medication compliance（RR=1.90）,reduced systolic pressure（MD=-8.16）,improved patient＇s blood pressure control（RR =1.53）and their health knowledge（RR =1.94）（all P〈0.05）.Conclusion Evidence-based nursing intervention can improve medication compliance and the awareness of hypertension,and have positive effect in controlling and stabling blood pressure.The further highquality studies are needed to reveal the evidence-based nursing intervention on the health status of patients with hypertension because the quality of included trials was low.
