多普勒超声检测腹腔动脉窃血现象与胃肿瘤的关系

摘　　要：目的:探讨彩色多普勒与脉冲多普勒超声检测胃肿瘤患者胃供血动脉的血流动力学变化及腹腔动脉窃血现象与胃肿瘤的关系。方法:对20例健康体检者、32例胃恶性肿瘤患者及25例胃良性肿瘤患者运用彩色多普勒、脉冲多普勒超声检测腹腔动脉、脾动脉、肝动脉、胃左动脉、腹主动脉收缩期峰值流速、舒张末期流速、内径、阻力指数、血流量、腹腔动脉与腹主动脉流量比值并对检测结果进行统计学分析。结果:(1)恶性组胃供血动脉收缩期峰值流速、内径、血流量及腹腔动脉与腹主动脉流量比值明显高于良性组及对照组(P<0.05),存在腹腔动脉窃血现象;(2)良性组胃供血动脉收缩期峰值流速、内径、血流量及腹腔动脉与腹主动脉流量比值与对照组比较,差异无统计学意义(P>0.05)。结论:胃供血动脉血流动力学改变及腹腔动脉窃血现象,可作为胃癌诊断及胃良、恶性肿瘤鉴别的重要诊断指标。Objective：To investigate the hemodynamic chang of the stomach to support blood vessels in pa-tients with gastric malignant carcinoma、gastric benign carcinoma and healthy persons;To investigate the relationship between celiac axis blood flow steal phenomenon and gastric tumor.Methods：A total of 77 ca-ses,32 cases with gastric malignant carcinoma,25 cases with gastric benign carcinoma,20 cases with health were detected with color Doppler and pulsed Doppler to measure the diameter、maximal、minimal velocities, resistance index and blood volume of the stomach to support blood vessels and abdominal aorta,calculate the ratio of the celiac artery to support blood vessels、Qc/Qa in gastric malignant carcinoma was higher than that of the gastric blood volume and abdominal aorta blood volume （Qc/Qa）.Results：Q、D、Vmax of the stomach benign carcinoma and the nomal group（P 〈0.05）,which indicate the flow steal phenomenon of celiac axis blood.Q、D、Vmax of the stomach to support blood vessels、Qc/Qa in gastric benign carcinoma compared with that of the nomal group was with that of the nomal group was not statistically significant（P〉0.05）.Conclusion：The hemodynamic chang of the stomach to support blood vessels and celiac axis blood flow steal phenomenon are helpful to offer diagnostic information for patients with gastric malignant carci-noma.
