摘 要:目的探讨临床路径应用于血液透析患者的健康教育干预效果。方法根据"不平衡指数最小的原则"将140例血液透析患者随机分为观察组和对照组,每组70例,对照组给予常规治疗和健康教育,观察组将临床路径应用于健康教育。干预后比较2组患者的生活质量、自我管理能力情况。结果干预前,观察组患者生理功能(58.2±18.1)等生活质量各维度评分,限制行为(20.6±3.2)等自我管理行为各因子评分与对照组差异均无统计学意义(P>0.05)。干预6个月后,观察组患者生理功能(68.9±16.9)、生理职能(37.3±15.2)等生活质量各维度评分、限制行为(23.7±3.3)、管理行为(11.1±2.7)等自我管理行为各因子评分均高于自身干预前评分及对照组评分,差异有统计学意义(P<0.05)。观察组患者定时复诊、自我监测、饮食控制、合理用药、运动治疗等遵医行为形成率分别为78.6%、81.4%、82.9%、94.3%和68.6%,均高于对照组,差异有统计学意义(P<0.01)。结论将临床路径应用于血液透析患者的健康教育,能提高患者的生活质量及自我管理能力。Objective To explore the effect of clinical pathway in health education intervention among hemodialysis patients. Methods According to unbalance index minimum principle,140 patients were randomly divided into intervention group( n = 70) and control group( n = 70),the clinical pathway was used in the observation group and conventional health education intervention was used in control group. After intervention,patients were investigated by self-filling questionnaire for quality of life and self-management skills on hemodialysis patients. Results There was no difference found in intervention group on quality of life in each dimension score such as physiological function( 58. 2 ± 18. 1),self-management behavior in each factor score such as restrictive behavior( 20. 6 ± 3. 2) compared to control groups( P〉 0. 05). After 6months intervention,quality of life in each dimension score such as physiological function( 68. 9 ± 16. 9),physical role( 37. 3 ± 15. 2),the self-management behavior in each factor score such as restrictive behavior( 23. 7 ± 3. 3),managerial behavior( 11. 1 ± 2. 7) in intervention group was significantly higher than that in control group( P〈0. 05). The percentages of regular review( 78. 6%),self-monitoring( 81. 4%),diet control( 82. 9%),rational drug use( 94. 3%) and sports therapy( 68. 6%) in intervention group were higher than that in control group( P〈 0. 01). Conclusion The clinical pathway applied to hemodialysis patients for health education intervention can significantly improve the quality of life and the level of self-management skills.
