摘　　要：目的:探讨经会阴超声检查在女性压力性尿失禁(Stress urinary incontinence,SUI)中的应用价值。方法:对2013年8月—2014年3月河北联合大学附属医院收治的79例SUI患者(轻度SUI组40例、中重度SUI组39例),同期因健康查体就诊后排除SUI和(或)盆腔器官脱垂的患者41例(对照组)行经会阴超声检查,在安静状态下测量膀胱尿道后角(α),在屏气用力动作时检测患者膀胱颈移动度(Mu),将两种状态下的图像冻结并存入计算机,之后测量膀胱颈旋转角度(θ)、膀胱颈下移距离(X)。结果:对照组与轻度SUI组、中重度SUI组间的Mu值、α角、θ角差异均有统计学意义(P<0.05),且与SUI的严重程度呈正相关(Mu、α、θ的r值分别为0.861,0.750,0.619,P<0.05)。对照组与轻度SUI组、中重度SUI组间的X值差异有统计学意义(P<0.05),而轻度SUI组与中重度SUI组间的X值差异无统计学意义(P>0.05)。结论:经会阴超声检查能够清晰、直观地显示盆底解剖结构及功能改变,在SUI的诊断中具有重要的应用价值。Objective： To explore the value of transperineal ultrasonography in female patients with stress urinary inconti- nence. Methods： Seventy-nine patients with stress urinary incontinence（40 patients in the mild group and 39 in the moderate to severe group） and 41 patients without SUI or POP in the control group were included from August 2013 to March 2014 in the affiliated hospital of Hebei United University. Perineal ultrasonography was performed to measure posteriorurethra-vesical angle （a） at rest and distance of bladder neck mobility （Mu） after Valsalva action. After taking and storing the images under such conditions, the rotation angle of vesical neck（θ） and the down-moving distance of bladder（X） were all measured. Results： The mean distance of Mu and angels of θ and ct were found to be statistically significant in the three groups （P〈0.05）, which were positively correlated with the severity of SUI （Mu, θ and ct values were 0.861, 0.750, 0.619, P〈0.05）. There was signifi- cant difference for mean distance X between the SUI group and control group （P〈0.05）, but no difference between the mild group and the moderate to severe group. Conclusion： Perineal ultrasonography can play an important role in diagnosing pa- tients with SUI by providing intuitive and clear changes of the anatomy and function of pelvic floor.
