血清抗C1q抗体在狼疮性肾炎患者的表达及其临床意义

摘　　要：目的研究血清抗C1q抗体在狼疮性肾炎(LN)及不同病理类型LN患者血清中的表达及临床意义。方法系统性红斑狼疮(SLE)患者97例分为合并LN(LN组,67例)和非LN(非LN组,30例);20例健康者为对照(NC组)。采用ELISA检测血清抗C1q抗体,分析抗体阳性率及与LN肾组织病理改变的相关性。结果 LN组血清中抗C1q抗体阳性率为59.7%,高于非LN组的10.0%和NC组的0(P均<0.01);其对LN诊断的敏感性为59.7%,特异性为94.0%。Ⅳ型LN抗C1q抗体阳性率77.8%,高于Ⅱ型的16.7%(P<0.01);LN抗C1q抗体对Ⅳ型LN诊断的敏感性为77.8%,特异性为81.5%。血清抗C1q抗体水平与肾脏病理活动指数(r=0.675,P<0.05)及SLE疾病活动指数(r=0.367,P<0.05)均呈正相关。结论在SLE患者血清中检测出抗C1q抗体,并在一定程度上反映了LN肾损情况和活动指数,在诊断和鉴别Ⅳ型LN中起了重要作用。Objective To investigate the expression and clinical significance of anti-C1q antibody in patients with lupus nephritis（LN）.Methods A total of 97 patients with systemic lupus erythematosus（SLE）was assigned into two groups of LN（with LN,67cases）and NLN（without LN,30cases）.Twenty healthy people were taken as the controls.Serum expression of anti-C1q antibody was detected by ELISA.Correlation of anti-C1q antibody expression and renal pathological changes was analyzed.Results The positive rate of anti-C1q antibody in group LN was 59.7%,which was higher than 10.0%in group NLN and 0%in group NC（P〈0.01）.The sensitivity and specificity for anti-C1q antibody to diagnose LN were 59.7% and 94%,respectively.The positive rate of anti-C1q antibody in type Ⅳ LN was 77.8%,which was higher than 16.7% in type Ⅱ LN（P〈0.01）.The sensitivity and specificity for anti-C1q antibody to diagnose type Ⅳ LN were 77.8% and 81.5%,respectively.Serum expression of anti-C1 q antibody was positively correlated with activity index of renal pathohistological changes（r=0.675,P〈0.05）and SLE disease activity index（r=0.367,P〈0.05）.Conclusion To some extent,serum level of anti-C1q antibody may reflect the renal lesion and active index,which takes an important role in diagnosing or identifying type Ⅳ LN.
