摘　　要：目的:探讨不同级别颅内血管外皮细胞瘤(HPC)的MRI特征,提高对该病的认识及术前诊断的准确性。方法:回顾性分析9例经手术和病理证实的不同级别颅内HPC的MRI征象。结果:9例患者中,8例位于颅内脑外,1例位于侧脑室内。WHOⅡ级HPC 5例,呈类圆形,边界清,瘤周水肿不明显,无相邻骨质破坏,2例与硬脑膜以窄基底相连,2例见"脑膜尾征";WHOⅢ级间变型HPC 4例,呈分叶状及不规则形,边界不清,瘤周水肿明显,有囊变、坏死,以窄基底与硬脑膜相连,2例相邻骨质破坏。肿瘤实性部分T1WI呈等或略低信号,T2WI为等或略高信号,DWI图像呈略高信号,内部囊变坏死呈长T1长T2信号,DWI图像呈低信号,增强扫描不均匀强化。7例瘤周或瘤内见流空血管影。结论:术前磁共振检查可为颅内HPC的临床诊断、治疗及判断预后提供帮助。Objective：To investigate the MRI characteristics of intracranial hemangiopericytoma（HPC）with different grading,in order to improve the knowledge of HPC and the diagnostic accuracy before surgery.Methods：The MRI features of 9HPC patients with different grading confirmed by operation and pathology were analyzed retrospectively.Results：Among the 9cases,8cases had intracraniao-extracerebral lesion and one patient had the mass located within lateral ventricle.5cases were graded as WHO ⅡHPC,which were oval in shape with clear margin,with neither significant peritumoral edema nor adjacent skull destruction.2of them had narrow base connected with meninges,and 2cases showed ＂ dural tail sign＂.4cases were graded as WHO Ⅲ anaplastic HPC,which were lobulated in shape with blurred margin,significant peritumoral edema,cystic degeneration and necrosis.All of the 4cases had narrow base connected with meninges.Adjacent skull destruction was found in 2cases.The parenchyma of the tumors showed isointensity or hypointensity on T1 WI,isointensity or hyperintensity on T2 WI and hyperintensity on DWI.While the cystic degeneration and necrosis within the tumors showed hypointensity on T1 WI,hyperintensity on T2 WI,hypointensity on DWI,and heterogeneous enhancement.Vascular flow void could be found within or around the tumor in 7cases.Conclusion：Preoperative MRI is beneficial for clinical diagnosis,treatment and prognosis of intracranial HPC.
