摘 要:目的探讨双可脱球囊技术栓塞治疗外伤性颈内动脉海绵窦瘘(traumatic carotid cavernous fistulas,TCCF)的方法及疗效。方法回顾性分析8例TCCF病人的临床资料,应用双导管将2个可脱球囊同时放入瘘口,分别充盈球囊后闭塞瘘口,依次解脱球囊,观察治疗效果。结果 8例病人均成功闭塞瘘口,同时保持颈内动脉通畅,未发生手术相关并发症。随访2~18个月,无一例复发。结论双可脱球囊技术安全、可靠,对于仅用单球囊无法闭塞瘘口的TCCF,双可脱球囊技术治疗提供一种新的治疗方法。Objective To investigate the method and therapeutic outcome of endovascular embolization with double-detachable-balloon technique in the treatment of traumatic carotid cavernous fistulas (TCCF). Methods Clinical data of 8 patients with TCCF were analyzed retrospectively. Two detachable balloons were advanced to the opening of the fistula through double tube catheters at the same time. The balloons were inflated to occlude the opening of the fistula respectively and were detached in turn. The clinical outcome was observed. Results The fistulas were occluded completely in 8 patients and internal carotid artery remained patent without the complications related to operation. There was no relapse during the followed-up period of 2 to 18 months. Conclusions The double-detachable-balloon technique was safe and reliable. The double-detachable-balloon technique provides a new choice of method for TCCF patients who can't be occluded by a single balloon.
