摘 要:目的运用组织工程学方法制作人工材料支架修复兔甲状软骨缺损,寻求一种重建甲状软骨的新途径。方法运用泡沫凝胶注模法制备多孔HA,以冷冻吸附法将Ⅰ型胶原、rh BMP-2沉淀于HA支架上形成rhBMP-2/Ⅰ型胶原/HA复合材料。实验动物分为两组,实验组以Ⅰ型胶原、HA、rh BMP-2为材料,对照组以Ⅰ型胶原、HA为材料,分别进行甲状软骨缺损的原位修复,观察并评估其修复效果。结果 HA体积分数为50%的多孔支架材料孔隙率为(50.54±3.02)%,孔隙孔径为100~200μm。大体观察见术后7周时,实验组支架材料降解完全,被片状骨性组织所替代,新生骨切面呈黄白色,质地较硬,且新生骨质与对侧未切除甲状软骨边缘紧密结合;对照组支架仍只见纤维结缔组织包膜。组织学检查见实验组术后3周材料降解,手术区域软骨母细胞增生伴成软骨区域形成;术后7周可见软骨细胞增生及软骨骨质形成。对照组3周、7周可见软骨缺损边缘软骨母细胞增生,少量成熟软骨细胞聚集,缺损中央区未见成熟软骨细胞。结论 rh BMP-2/Ⅰ型胶原/HA复合材料具有良好生物相容性,是骨诱导和骨传导性好的材料,可作为一种新型甲状软骨缺损的修复材料。Objective To explore a new method for in situ remediation of thyroid cartilage defects with artificial material scaffolds made by tissue engineering method.Methods Porous hydroxyapatite( HA) was prepared with foam gel injection molding method.And then collagen I and rh BMP-2 were precipitated on the HA scaffold by frozen vacuum adsorption method,forming collagen I / rh BMP-2 / HA composite.Experimental animals were divided into two groups,collagen I,HA and rh BMP-2 were used for repair of thyroid cartilage defects in the experimental group while collagen I and HA in the control group.The remediation effects were assessed by general observation and histological examination.Results The bracket porosity in HA volume fraction of 50% was 50.54% ± 3.02% and the pore diameters ranged from 100μm to200μm.Gross observation 7 weeks after surgery showed complete degradation of scaffold materials,which were replaced by sheets of indurated osseous tissue with yellowish-white section,in the experimental group.The new bone was integrated tightly with the edge of contralateral plate of thyroid cartilage.In the control group,there were only fibrous connective tissue capsules in the scaffolds.Histological examination at the 3rdweek after surgery showed material degradation,chondroblastoma hyperplasia with formation of chondrogenic area in the surgical field in the experimental group.The examination at the 7thweek after surgery showed visible chondrocyte hyperplasia and cartilage formation.While in the control group,examination at the 3rdand 7thweek showed chondroblastoma hyperplasia with small aggregation of cartilage cells only at the border but not center of the defect.Conclusion With advantages of good biocompatibility,bone induction bone conductibility,Collagen I / rh BMP-2 / HA composite material is a new material for repair of thyroid cartilage defect.
