The Correlation of Amniotic Fluids Levels and Blood Serum Levels of Matrix Metalloproteinases-8 and Interleukin-6 with Preterm Delivery

摘  要:目的研究羊水中基质金属蛋白酶(MMP)-8和白细胞介素(IL)-6在早产临产患者中的发生率及其意义。方法选择于2010年1月-2012年12月分娩的单胎初产妇200例作为研究对象,按分娩时孕周分为早产临产组(n=102)和足月临产组(n=98)。用酶联免疫吸附试验分别检测孕妇羊水和血清中MMP-8及IL-6的表达,同时行羊水细菌培养。结果早产临产组与足月临产组孕妇羊水中MMP-8水平分别为(320.45±59.88)、(153.72±29.12)ng/m L,差异有统计学意义(P<0.05);但早产临产组与足月临产组孕妇血清MMP-8水平分别为(9.56±2.11)、(9.42±2.01)ng/m L,差异无统计学意义(P>0.05)。早产临产组与足月临产组孕妇羊水IL-6水平分别为(90.5±16.3)、(20.6±12.5)μg/L,血清IL-6水平分别为(159.2±20.4)、(22.3±11.8)μg/L,组间差异均有统计学意义(P<0.05)。早产临产组与足月临产组孕妇羊水中致病菌培养阳性率分别为8.8%(9/102)、1.0%(1/98),差异有统计学意义(P<0.05)。结论宫内感染在早孕临产组孕妇中比例较高,是导致早产的主要因素;羊水MMP-8水平的升高、羊水及血清中IL-6水平的升高对预测早产有一定临床应用价值,尤其是血清IL-6水平升高可以作为一项临床应用开展。Objective To investigate the expression of amniotic fluid levels and blood serum levels of matrix metalloproteinases-8(MMP-8) and interleukin-6(IL-6) in women with preterm delivery. Methods Between January 2010 and December 2012, we collected the amniotic fluid of 102 preterm pregnant women and 98 full term pregnant women and analyzed the MMP-8 levels and IL-6 levels in amniotic l uid and blood serum. Meanwhile, we also collected the amniotic fluid to do bacterial culture. Results The amniotic fluid levels of MMP-8 in preterm pregnant women were higher than those in full term pregnant women [(320.45±59.88) vs(153.72±29.12) ng/m L, P 〈 0.05], but there was no obvious discrepancy in the blood serum levels of MMP-8 in the two groups [(9.56±2.11) vs(9.42±2.01) ng/m L, P 〉 0.05]. BotThamniotic l uid levels and blood serum levels of IL-6 in preterm pregnant women were significantly higher than the full term pregnant women [(90.5±16.3] vs(20.6±12.5) μg/L, P 〈 0.05;(159.2±20.4) vs(22.3±11.8) μg/L, P 〈 0.05]. The positive bacterial culture rate of preterm pregnant women was higher than the full term pregnant women(8.8% vs 1.0%,P 〈 0.05). Conclusions Infection is the most important reason for preterm pregnancy. MMP-8 level increases in the amniotic l uid, and the level of IL-6 in amniotic l uid and blood serum is a valuable clinical index for identifying premature delivery.
