移植术后应用他克莫司致不良反应的相关因素分析

摘　　要：目的:了解移植术后应用免疫抑制他克莫司致不良反应(ADR)发生的特点及相关因素,为临床安全应用他克莫司提供参考。方法:分别以"他克莫司""FK506""普乐可复""不良反应""移植"组合关键词,检索中国期刊全文数据库(CNKI)、万方数据库以及维普数据库中收载的他克莫司致ADR文献29篇,共46例病例,并进行统计、分析。结果:移植术后应用他克莫司所导致的ADR,可能与患者的性别、年龄、给药途径与剂量、治疗阶段、移植类型、血药浓度、合并用药等多个因素相关。结论:为减少移植术后他克莫司ADR的发生,临床应综合考虑患者非遗传因素(年龄、给药途径与剂量、治疗阶段等)和遗传因素的影响,制订个体化给药方案,确保其安全、合理应用。OBJECTIVE：To study the characteristics and related factors of adverse drug reaction（ADR）induced by tacrolimus after transplantation,and provide reference for the clinical application of tacrolimus. METHODS：In total of 29 literatures including 46 cases ADR induced by tacrolimus were statistically analyzed,which were from CNKI database,Wanfang database and VIP database with the combination keyword of＂Tacrolimus＂,＂FK506＂,＂Prograf＂,＂ADR＂and＂transplantation＂. RESULTS：The incidence of ADR was related to the age and sex of patients,dosage of administration,stages of treatment,types of transplantation,plasma concentration and combination therapy,etc. CONCLUSIONS：In order to reduce the incidence of ADR of tacrolimus after liver transplantation,non genetic factors（age,dosage,treatment stage,etc.）and genetic factors should be taken into consideration to set individualized dosage regimen to ensure its safe and reasonable application.
