单纯静脉血营养的腕掌侧游离静脉皮瓣修复指背软组织缺损疗效分析

摘　　要：目的探讨单纯静脉血营养的腕掌侧游离静脉皮瓣在修复指背软组织缺损的疗效。方法回顾分析收治的手指背侧软组织缺损患者30例,采用切取腕掌侧静脉游离皮瓣与指背侧静脉吻合修复创面,观察术后手指功能、皮瓣两点辨别觉、皮瓣色素及弹性,随访8个月观察皮瓣存活及功能情况。结果皮瓣存活28例,1例部分坏死,1例完全坏死。患指活动功能按TAM评分,优:82%,良:10%,中5%,差:3%;皮瓣两点辨别觉平均(9±0.5)cm,色素沉着中度,弹性良好。结论单纯静脉血营养的腕掌侧游离静脉皮瓣在修复指背皮肤缺损手术易于操作,相比带蒂皮瓣对手指外观影响较小,特别是伴有伸肌腱缺损时可同时移植掌长腱,效果良好,是一种可靠的临床修复方法。Objective Analysis of the curative effect of the carpometacarpal side venous flaps nourished by venous blood to repair dorsal soft-tissue defects of finger. Methods To retrospective analysis 30 cases of dorsal soft tissue defects of finger repaired by the carpometacarpal side venous flaps nourished by venous blood, between july 2010 and october 2012. During 8 months of follow-up, to observe the following indicators： hand function, two-point discrimination, pigmentation and elasticity. Results Total survival occurred in 28 cases. Partial necrosis occurred in 1 case; total necrosis occurred in 1 case. According to finger function scoring system of TAM, the results were excellent rate in 82%, good rate in 10% ,middle rate in 5M, poor rate in 3%. The two-point discrimination was mean 9±0. 5cm. Results All flaps survived,less pigmentation and good elasticity. Conclusion The carpometacarpal side venous flaps nourished by venous blood to repair dorsal soft-tissue defects of finger is easy to operate,comparision of pedicle flap over a smaller impact on the appearance of the fingers,also on graft palmaris longus tendon will acquires favourable effects. So, this way is a reliable way.
