磁共振定位CT导引前列腺穿刺活检的临床应用

摘　　要：目的探讨磁共振(MRI)定位CT导引下前列腺穿刺活检的临床应用价值。方法对25例疑有前列腺病变的患者,采用MRI多序列成像确定病变可疑区,再行经会阴CT导引下穿刺活检,取材送病理检查明确诊断,所得病理结果与术后病理结果对照比较。结果 25例患者穿刺活检结果为前列腺癌7例(28.0%),不典型性腺瘤样增生及前列腺上皮内瘤变8例(32.0%),良性前列腺增生10例(40.0%);外科术后病理结果为前列腺癌8例(32.0%),不典型性腺瘤样增生及前列腺上皮内瘤8例(32.0%),良性前列腺增生9例(36.0%);穿刺活检与病理检查比较差异无统计学意义(P>0.05)。结论采用磁共振多序列成像定位后CT导引下前列腺可疑结节穿刺活检,可为临床医生在前列腺疾病的诊断、鉴别诊断和治疗提供可靠依据。其阳性率高、操作安全、并发症少、患者痛苦轻。Objective To explore the clinical value of magnetic resonance located and CT guided needle biopsy of prostate cancer. Methods Lesion suspicious area of 25 patients with clinical suspicion of prostate cancer were determined by MRI sequence imaging, perineum CT was used to guide prostate biopsy. The biopsy specimen were analysed based on pathologic examination, the pathological results were compared with postoperative pathologic results. Results The needle biopsy of 25 patients tests showed that 7 patients had prostate cancer（28%）, 8 patients had atypical adenomatous hyperplsia and prostatic intraepithelial neoplasia （32%）, 10 patients had benign prostatic hyperplasia （40 %）. The pathological results of surgery showed that 8 patients had prostate cancer（32%）, 8 patients had atypical adenomatous hyperplasia and prostatic intraepithelial neoplasia （32%）, 9 patients had benign prostatic hyperplasia （36%）. Differences between the two groups has no statistical significance （P〉0.05）. Conclusion By MRI multi-sequence image positioning, CT guiding suspicious nodules of prostate biopsy, with high positive rate of prostate cancer diagnosis, safer operation and fewer pain, can provide reliable basis for the diagnosis, differential diagnosis and treatment of prostate disease.
