适当低浓度对比剂在冠状动脉CTA成像中的应用价值

摘　　要：目的探讨适当低浓度对比剂在冠状动脉CTA成像中的应用价值。方法对136例拟行冠状动脉CTA检查的患者行前瞻性研究,按所用对比剂浓度不同分为三组,所用对比剂浓度分别为320 mg I/ml、350 mg I/ml、370 mg I/ml,将扫描所得原始数据进行后处理重组,然后对重组后的冠状动脉图像质量进行主观评价,并对主动脉起始部及同层面降部的CT值进行测量,对其结果分别进行统计学分析。结果三组冠状动脉图像质量的主观评价差异无统计学意义(χ2=0.785,P=0.675)。采用前瞻性心电门控扫描的三组患者主动脉起始部CT值的分析结果为:F值=0.992、P值=0.376;主动脉降部CT值的分析结果为:F值=1.527、P值=0.224,采用回顾性心电门控扫描的三组患者主动脉起始部CT值的分析结果为:F值=1.206、P值=0.307;主动脉降部CT值的分析结果为:F值=1.703、P值=0.191,各组CT值的差异无统计学意义。结论适当低浓度对比剂(320 mg I/ml)能够满足冠状动脉CTA的诊断要求,在行冠状动脉CTA检查时推荐使用。Objective To investigate the application of proper low-concentration contrast agent in coronary CT angiography Methods This prospective study was carried out in 136 patients who were scheduled to receive coronary CT angiography.According to the concentration of the contrast used in angiography,the patients were divided into three groups： 320 mg I / ml group,350 mg I / ml group and 370 mg I / ml group.The raw data obtained from the scanning were reconstructed through post-processing,then the image quality was subjectively evaluated,CT values at the level of initial part of the aorta and the descending aorta at the same level were determined,the results were statistically analyzed.Results No statistically significant differences in subjective scoring of coronary artery image quality existed between each other among the three groups（ P 〉 0.05,χ2= 0.785,P = 0.675）.Using prospective EKG-gating scanning,the analysis result of CT values determined at the initial part of the aorta was F = 0.992 and P = 0.376; the analysis result of CT values determined at the descending aorta was F = 1.527 and P = 0.224.Using retrospective EKG-gating scanning,the analysis result of CT values determined at the initial part of the aorta was F = 1.206 and P = 0.307; the analysis result of CT values determined at the descending aorta was F = 1.703,P = 0.191; in all three groups the analysis result of CT values determined at the descending aorta was F = 1.703 and P = 0.191,and no statistically significant differences in CT values existed between each other among the three groups.Conclusion Coronary CT angiography using proper low-concentration contrast（ 320 mg I / ml）can meet the diagnostic requirement.Contrast concentration of 320 mg I / ml is recommended for coronary CT angiography.
