摘 要:目的探讨头颈部256层CTA低剂量扫描的可行性。方法将84例行头颈部CTA检查的成人(体质指数(BMI)<22.9 kg/m2)随机分为常规剂量组(400 m As组)、300 m As组和200 m As组,每组28例,均采用Philips 256层CT扫描,3组管电压均为120k V,管电流分别为400 m As、300 m As和200 m As,对照分析3组的图像质量及辐射剂量。结果 3组主观图像质量评分及颈总动脉(CCA)与基底动脉(BA)平均CT值比较差异无统计学意义(P>0.05)。200 m As组的背景噪声(N)明显高于常规剂量组和300 m As组(P<0.05),信噪比(SNR)明显降低(P<0.05)。3组的CT剂量指数(CTDIvol)和剂量长度乘积(DLP)随管电流降低而逐步降低(P<0.05)。结论头颈部256层CTA检查管电压120 k V、管电流300 m As时的图像质量好、辐射剂量较低,可作为最佳扫描参数。Objective To evaluate the feasibility of low-dose 256-slice spiral CT angiography( CTA) in diagnosing head and neck lesions.Methods A total of 84 adult patients( BMI 〈 22.9 kg / m2) were enrolled in this study.Cephalocervical CTA was performed in all patients.According to the tube current used in scanning,the patients were randomly divided into routine dose group( 400 m As group,n = 28),300 m As group( n = 28) and 200 m As group( n = 28).Philips256-slice spiral CT scanner and tube voltage of 120 k V were used for all patients.The image quality and radiation dose were determined,and the results were compared between each other among the three groups.Results No statistically significant differences in the subjective image quality score and the average CT value of CCA and BA existed between each other among the three groups( P 〉 0.05).The noise of 200 m As group was significantly higher than that of routine dose( 400 m As) group and 300 m As group( P 〈 0.05),while SNA of 200 m As group was significantly lower than that of routine dose( 400 m As) group and 300 m As group( P 〈 0.05).The CTDIvol and DLP of all three groups were gradually decreased along with the decreasing of tube current( P 〈 0.05).Conclusion 256-slice spiral CT cephalocervical angiography using 120 k V and 300 m As has lower radiation dose with good quality images,and 120 k V and 300 m As can be used as the optimal scan parameters.
