摘　　要：目的：探讨急性间质性肺炎（AIP）的 HRCT表现。方法回顾性分析经穿刺肺活检或临床综合诊治证实的4例 AIP 的连续 HRCT影像资料。结果 AIP的早期影像表现为两肺外带对称性分布磨玻璃影，可累及叶间胸膜及纵隔胸膜下肺组织；中期主要表现为两肺弥漫分布磨玻璃影、实变影，出现小叶间隔及小叶内间隔增厚；晚期主要表现为肺结构扭曲，牵拉性支气管扩张、蜂窝影形成、肺气肿、气胸、胸腔积液等。结论 AIP特征性的影像学表现进展迅速，以急进性的肺纤维化和肺结构的破坏为特征性改变。Objective To discuss the imaging features of acute interstitial pneumonia （AIP）.Methods To retrospective analysis the continuous CT image data of 4 cases proved AIP by open lung biopsy or clinical comprehensive diagnosis.Results Early manifestations of AIP were bilateral lung periphery symmetrical distribution of ground glass opacity,involving interlobar pleural and mediastinal subpleural lung tissue.The middle phase of AIP were bilateral pulmonary diffused ground glass opacity ,consolidation,appeared the interlobular septa and intralobular septal thickening.The imaging findings in the late phase were traction bronchiectasis,honeycombing,emphysema, pneumothorax,pleural effusion.Conclusion AIP characteristic imagines progress rapidly.The characteristic changes of AIP are pulmonary interstitial fibrosis and lung structure rapidly progressive destruction.
