MSCT在诊断非特异性肾周围脓肿中的临床价值

摘　　要：目的：探讨非特异性肾周围脓肿的多层螺旋CT（MSCT）表现。方法回顾性分析9例经临床治疗证实的非特异性肾周围脓肿患者的CT表现。9例患者首次CT检查均行CT平扫及增强扫描，治疗后行CT平扫复查。结果 9例肾周围脓肿中，右侧4例，左侧5例。病变包绕患侧肾脏，与患侧肾完全分界不清者2例，部分分界不清者7例，累及患侧腰肌、腹后壁2例。增强后病灶环状强化，其中大小均匀的多发小脓腔2例，以大单房脓腔改变为主边缘见小的脓腔5例，以多发不规则大的多房状脓腔改变2例。结论 MSCT对肾周围脓肿能明确诊断，可充分显示肾周围脓肿的范围、内部分隔、周围组织受累情况。Objective To evaluate the features and clinical value of multi-slice CT in the diagnosis of non-specific perinephric abscess. Methods MSCT features of 9 patients with perinephric abscess were analyzed retrospectively.All of the patients underwent plain and enhanced scanning at first CT examination,and underwent plain CT scan after treatment.Results Of all the 9 patients,perinephric abscesses were found on the right side in 4 cases and left in 5 cases.The lesions surrounded the kidney with entirely unclear boundary in 2 cases and partly unclear boundary in 7 cases.The psoas muscle,posterior abdominal wall were involved in 2 cases.All of the lesions demonstrated ring enhancement.Multiple small uniform abscesses were seen in 2 cases.Large single-cavity abscess surrounded by multiple small abscesses were seen in 5 cases.Multiple large irregular multi-cavity abscesses were seen in 2 cases.Conclusion MSCT can diagnose the non-specific perinephric abscesse,sufficiently displays the range of the abscess,inner compartment,involvement of the surrounding tissue.
