中药静脉制剂干预急性心肌梗死的系统评价

摘　　要：目的:评价中药静脉制剂干预急性心肌梗死的疗效和安全性。方法:系统检索Cochrane图书馆、Pub Med、EMbase、CBM、CNKI、VIP等中外生物医学数据库(截至2013年12月),全面收集有关中药静脉制剂治疗急性心肌梗死的临床随机对照试验(RCT),文献质量评价参照Cochrane系统评价员手册。结果:本研究共纳入21项研究,共计1 713例患者,结局指标为病死率,冠脉再通率,并发症的发生率(心律失常、休克、心衰)等。由于纳入研究间异质性较大,没有进行定量合并分析,只做描述性分析。结论:中药静脉制剂显示出降低急性心肌梗死病死率,减少并发症的作用,但是由于本研究纳入研究方案差异大,质量不高,故中药静脉制剂对急性心肌梗死的疗效尚缺乏可靠证据的支持,有待于更多高质量研究的开展以进一步证实其疗效和安全性。Objectives： To evaluate the curative effect and security of traditional Chinese medicine injections in intervention of myocardial infarction.Methods： Randomized controlled clinical trials（RCTs） about treating myocardial infarction with traditional Chinese medicine injections were collected by searching Cochrane library,Pub Med,EMbase,CBM,CNKI and VIP ending Dec.2013.Literature quality was evaluated according to Cochrane system evaluation manual.Results： 21 RCTs and 1 713 patients were included in this study,and the outcome indicators contained mortality,reperfusion rate,complications incidence（arrhythmia,shock and heart failure） and so on.This study only made description analysis and without a quantitative analysis on the results because of the larger heterogeneity among included studies.Conclusion： Traditional Chinese medicine injections could reduce the mortality of acute myocardial infarction,reduce complication,while the difference among research plans included in this study is large and the literature quality of this study is low,so it lack reliable evidence to support the curative effect of treating acute myocardial infarction with traditional Chinese medicine injections,and it needed to develop more high quality researches to confirms its efficacy and security.
