中西医联合干预经皮肾镜取石围手术期预防全身炎症反应综合征的疗效

摘　　要：目的探讨中西医联合干预、预防或减轻经皮肾镜取石围手术期全身炎症反应综合征的效果。方法模拟人体进行微通道经皮肾镜取石术围手术期处理和手术过程,选择雄性新西兰大白兔制作动物模型,实验组20只,对照组5只;2组均灌服盐酸左氧氟沙星胶囊25 mg,1日1次;于术后0.5 h开始,实验组静脉注射痰热清注射液0.3~0.5 m L·（kg·d）-1,1日1次,均连用7 d,对照组注射等量0.9%氯化钠。选不同时间段抽取外周静脉血,监测白细胞介素-2（IL-2）、白细胞介素-6（IL-6）、肿瘤坏死因子（TNF）及甲状腺激素（TT4、TT3）的变化。结果 IL-2、TT4、TT3的变化是先降后升,而IL-6、TNF是先升高后回落;实验组恢复程度明显好于对照组。结论中西医联合干预经皮肾镜取石围手术期对预防或减轻全身炎症反应综合征起积极的作用。Objective To explore effects of traditional Chinese medicine and western medicine combined for prevent or reduce the systemic infla-mmatory response syndrome （ SIRS） in peri-operation period of percuta-neous nephroscope.Methods The microchannel percutaneous nephro-lithotomy perioperative management and operation process in peri -operation period of human were simulated.The male white rabbits of New Zealand were selected to produce animal model , which were divided into test group （ n=20 ） , control group （ n=5 ）.The rabbits were given oflo-xacin hydrochloride 25 mg every time, once a day for 7days in two groups.And then the rabbits in test goup were intravenously the Tan-reqing Injection 0.3~0.5 mL · （ kg · d ） -1 every time , once a day for 7days.The level of interleukin 2 （IL-2）, interleukin 6 （IL-6）, tumor necrosis factor （ TNF） , thyroid hormone （ TT4 , TT3 ） were detected from peripheral venous blood of the rabbits on multiple time periods （ preoper-ative, postoperative 1 day, 3 days and 7 days）.Results The level of IL -2, TT4 and TT3 were found to drop significantly at first and to re-bound afterwards , while the concentration of IL -6 and TNF were de-tected to rise at first and fall back afterwards.The effect of the drugs in＆amp;nbsp;test group was found to be better than the control group significantly.Conclusion Traditional Chinese and western medicine combined intervention take a positive role in preventing or reducing the SIRS.
