摘　　要：目的：探讨Ⅰ代动脉导管未闭封堵器（PDA Occluder I）应用于膜部室间隔缺损（pm VSD）封堵术的安全和可行性。方法：2007年7月至2014年5月,18例（男性6例,女性12例）pm VSD患者接受封堵治疗,14例单独应用1枚PDA封堵器,4例同时应用1枚PDA封堵器和1枚VSD封堵器。患者年龄3.5~51岁,中位数17岁,随访时间3~72个月。对封堵成功率,术后相关并发症、超声心动图、心电图检查结果以及住院时间进行总结分析。结果：18例pm VSD均合并膜部瘤,13例右心室面单发破口,5例2处或多发破口。18例患者均手术成功,所有患者术后随访无严重心律失常等并发症发生;2例封堵术后少量残余分流,1例随访至第12个月残余分流消失。结论：Ⅰ代PDA封堵器应用于部分选择性合并膜部瘤的pm VSD,短-中期随访效果满意,远期效果尚需进一步观察。Objective：The aim of this report is to describe our experience with closure of perimembranous ventrieular septal defects （pmVSD） using the Amplatzer Duct Occluder I （ADO I）. This study reports our short-medium term follow up of pmVSD closure using the ADO ] . Methods： From June 2007 to May 2014, we used ADO I in 18 pmVSD patients. There were 5 male and 13 female patients with an age range of 3.5 -51 years with a median of 17 years. In this group, all these pmVSD had different shape of membranous aneurysm. Main outcome measures： The success rateaverage, rates of complications and readmissions, results of Echocardiography/ECG and the length of hospital stay. Results：All the pmVSDs are associated with an aneurysm that protrudes into the right ventricle. There were no major complications and immediately after the procedure and all the patients remained in normal sinus rhythm. The patients had 3 - 72 months with a median of 12 months follow up and 2 of these patients had mild residual shunt, and one disappeared at 12 months after the procedure, one remained unchanged at this latest follow up. Conclusion： ADO I is feasible and a safe device for a part of pmVSD with membranous aneurysm, short-medium results are good. Further clinical trials are underway to assess the long-terra results.
