他汀类药物治疗外周动脉粥样硬化的疗效比较

摘　　要：目的:通过观察颈动脉粥样硬化斑块和踝肱指数情况,探讨他汀类药物治疗外周动脉粥样硬化的临床疗效。方法:将180例急性脑梗塞患者随机分为瑞舒伐他汀组(60例)、阿托伐他汀组(60例)和常规治疗组(60例)。瑞舒伐他汀组每晚口服瑞舒伐他汀片10 mg,阿托伐他汀组每晚口服阿托伐他汀片20 mg,对照组应用其他标准缺血性脑卒中治疗。于用药前、用药后6个月,检测观察并记录血脂变化、颈动脉粥样硬化斑块和踝肱指数情况。结果:治疗6个月后,瑞舒伐他汀组与阿托伐他汀组颈动脉IMT与治疗前比较均有统计学差异(P均<0.5),瑞舒伐他汀组ABI定量水平高于同组治疗前(P<0.05);两组均未发生肝功能异常及横纹肌溶解。结论:瑞舒伐他汀与阿托伐他汀均能够稳定颈动脉粥样硬化斑块,瑞舒伐他汀强化治疗外周动脉粥样硬化疗效较好。Objective： To investigate the clinical efficacy of different statins for peripheral artery atherosclerosis by observing the carotid atherosclerotic plaques and ankle-brachial index. Methods： One hundred and eighty patients with acute cerebral infarction were randomly assigned to rosuvastatin group（ 60）,atorvastatin group（ 60）,and control group（ 60）. During the treatment,the patients in rosuvastatin group were treated with rosuvastatin tablets 10 mg every night for 6 months. The patients in atorvastatin group were treated with atorvastatin tablets20 mg every night for 6 months. The patients in the control group were treated with other standard treatments for ischemic stroke. The patients of the three groups were detected for the levels of blood lipid,the variance condition of carotid atherosclerotic plaques and ankle-brachial index before and 6 months after treatment. Results： There was statistically significant difference in the carotid artery IMT of the rosuvastatin group and atorvastatin group after 6months of treatment compared to pre-treatment level（ P〈0. 05）. In rosuvastatin group,the ABI quantitative level was higher than that before treatment（ P〈0. 05）. Liver function abnormalities and rhabdomyolysis did not occur in the two groups. Conclusion： Rosuvastatin and atorvastatin can stabilize carotid atherosclerotic plaques. And rosuvastatin enhances treatment of peripheral atherosclerosis more effectively.
