低温等离子射频消融术在儿童舌根囊肿的应用

摘　　要：目的探讨低温等离子射频消融术在儿童舌根囊肿的可行性及优越性。方法在全身麻醉气管内插管的条件下,采用低温等离子射频消融术对本科2010年1月～2013年6月收治的76例儿童舌根囊肿患者进行治疗,记录手术时间、出血量以及并发症,并进行随访。结果 76例患儿囊肿均完全切除,其中70例患儿在全身麻醉下进行了一次性治疗,6例患儿因呼吸困难,在床头无麻醉直接喉镜下进行第1次穿刺抽液术,待呼吸困难缓解,生命体征平稳后在全身麻醉下进行了第2次手术治疗。患儿无明显咽痛、出血等并发症,语音和舌根运动功能不受影响。随访6个月～3年,仅1例术后半年复发,治愈率为98.68%（75/76）。结论低温等离子射频消融术治疗舌根囊肿具有创伤小、术后并发症少和复发率低的优点,为临床提供了一种新的治疗手段。Objective To explore the feasibility and advantage of low-temperature radiofrenquency ablation for the treatment of tongue base cyst. Methods Low-temperature radiofrequency ablation was used to treat tongue base cyst of 76 patients in under general anesthesia with endotracheal intubation,admitted from Jan. 2010 to Jun. 2013. The operation time,the volumn of blood loss and postoperative complications were recorded and the follow-up was taken after the surgery. Results The tongue base cysts were totally resected in all patients,among which 70 patients only underwent one operation under general anesthesia and 6 patients received emergency treatment of cyst puncture bedside under no anesthesia because of dyspnea. There were no significant postoperative haemorrhage,pharyngalgia,dyspnea or other complications. Speech and the motor function of tongue base were not affected. Follow-up survey ranged from 6 months to3 years and the cure rate was about 98. 68%（ 75 /76）. The recurrence occurred only in one case. Conclusions Lowtemperature radiofrequency ablation is a promising management for the tongue base cyst with less trauma,complications or recurrence.
