摘 要:目的探讨丹参多酚对慢性阻塞性肺疾病急性加重期肺动脉压及D-二聚体的影响。方法选取慢性阻塞性肺疾病患者110例,随机分为对照组和观察组,两组患者均接受抗感染、平喘、化痰、吸氧等治疗,观察组患者在此基础上再给予丹参多酚200mg/d静脉滴注。两组的疗程均为2周,分别测定并比较两组患者治疗前后的肺动脉压和D-二聚体水平。结果治疗后两组患者D-二聚体水平均有下降,其中观察组下降更为明显,与治疗前比较,差异有统计学意义(t=2.0111,P〈0.05);经治疗后,两组患者的肺动脉压均下降,其中观察组治疗后与治疗前比较差异有统计学意义(t=5.3682,P〈0.01),与对照组治疗后相比,差异亦有统计学意义(t=2.8899,P〈0.01)。结论丹参多酚可有效改善慢性阻塞性肺疾病患者的血液高凝状态,改善机体的氧合功能,降低肺动脉压,值得在临床上推广应用。Objective To investigate the effect of salviandate on pulmonary artery pressure and D-dimer of in patients with acute exacerbated chronic obstructive pulmonary disease. Methods The clinical data of 110 patients with acute exacerbated chronic obstructive pulmonary disease were retrospectively analyzed. All the patients were randomly divided into control group and treatment group. Both groups accepted anti-infection, phlegm, asthma, oxygen therapy etc. The treatment group was given salvianolate 200 mg/d, intravenous infusion more. The treatment lasted for 14 days, pulmonary artery pressure and D-dimer were measured before and after treatment, the results were statistically analyzed. Results Before treatment, patients with acute exacerbated chronic obstructive pulmonary disease D-dimer levels were higher than the upper limit of normal reference values, D-dimer levels in the two groups after the treatment were decreased, it was decreased more significantly before and after treatment in the treatment group (t = 2. 0111 ,P 〈 0.05). The pulmonary arterial pressure were decreased in the two groups after the treatment, it was decreased more significantly in the treatment group before and after treatment ( t = 5. 3682, P 〈 0.01 ) ; and after treatment, there were significant differences between the two groups ( t = 2. 8899, P 〈 0. 01 ). Conclusions Salvianolate injection can improve the hypercoagulable states, improve oxygenation and reduce pulmonary artery pressure in patients with acute exacerbated chronic obstructive pulmonary disease, helps to improve the clinical efficacy.
