上海市级医院日间手术发展的影响因素研究

摘　　要：采用PEST分析法对上海市级医院日间手术发展宏观环境进行分析,并利用鱼骨法对上海市级医院日间手术发展的促进和阻碍因素进行阐述,指出促进因素包括:日间手术理念的推广、医学技术以及医院信息化的发展、医院发展的内在驱动因素影响、医院绩效考核的引导、医院上级管理部门和医保部门的考核要求等;阻碍因素包括:传统管理思维和固有认知习惯的局限、硬件设施限制、专业人员的不足、医保支付限制、服务规范性有待提高、社区康复体系不完善等。最后重点分析了国外日间手术发展的关键影响因素,以期实现对比借鉴。This study used PEST method to have a global view of ambulatory surgery in Shanghai Municipal Hospitals, and used fishbone diagram method to analyze the stimuli and hindrance factors specifically. It shows that the advantage factors includes the promotion of ambulatory surgery concept, medical technology and medical information technology development, hospital performance development and the inspection by health bureau and medical insurance bureau. The disadvantage factors includes the barriers of existing management concept, lack of professionals, the constraints of medical insurance payment, lack of service standards and the incompetence of community health system. With the intention of providing useful reference, we mainly analyzed the key influencing factors of ambulatory surgeryin abroad.
