摘 要:目的分析本院泌尿系感染患者的病原菌分布及耐药性,为临床泌尿系统感染的预防和治疗提供实验依据。方法对中段尿培养分离的259株细菌进行菌株鉴定和药敏试验,大肠埃希菌和肺炎克雷伯菌进行产超广谱β-内酰胺酶(ESBLs)检测。结果 259株病原菌中革兰阴性杆菌187株,占72.2%(187/259),革兰阳性菌43株,占16.6%(43/259),真菌29株,占11.2%(29/259)。革兰阴性菌中以大肠埃希菌(139株)和肺炎克雷伯菌(22株)为主,产ESBLs的大肠埃希菌和肺炎克雷伯菌检出率分别为55.4%(77/139)和45.4%(10/22);药敏结果显示,革兰阴性杆菌对亚胺培南、美罗培南敏感性较好(100%);革兰阳性球菌对万古霉素、替考拉宁、利奈唑烷敏感性较高(100%)。结论泌尿系感染中以革兰阴性杆菌为主,大肠杆菌为最常见的病原菌,未发现耐万古霉素的革兰阳性球菌。根据药敏结果合理选用抗菌药物对泌尿系感染的治愈与耐药菌株的控制有重要意义。Objective To analyze the pathogen distribution and drug resistance in the patients with urinary system infection in our hospital so as to provide the basis for prevention and treatment of urinary system infection.Methods 259 strains of pathogenic bacteria cultured and isolated from the midstream urine were performed the bacteriologic identification and the drug susceptibility testing,Escherichia coli and Klebsiella pneumoniae were performed the extend-spectrumβ-lactamase(ESBLs)producing testing.Results Among 259 strains of bacteria,187 strains 72.2%(187/259)were Gram negative stains,43 strains 16.6%(43/259)were Gram positive stains and 29 strains 11.20%(29/259)were fungi.Gram negative stains were dominated by Escherichia coli(139strains)and Klebsiella pneumoniae(22strains).The detection rates of ESBLs-producing Escherichia Coli and Klebsiella pneumoniae were 55.4%(77/139)and 45.4%(10/22).The drug susceptibility testing results showed that Gram negative stains were susceptible to both imipenem and meropenem(100%)and Gram positive stains were susceptible to vancomycin,teicoplanin and linezolid(100%).Conclusion Gram negative bacilli are the main infectious pathogens in urinary tract infection and Escherichia coli is the most common pathogen.No vancomycin-resistant Gram positive cocci is found.Rationally selecting antibacterial drugs according to the drug susceptibility testing results has great significance in the therapy of urinary tract infection and the control of drug-resistant bacterial strains.
