摘 要:目的 :通过对比两种方法勾画的内肿瘤区(IGTV)位置和体积差异,探讨一种满足临床准确度要求且高效的4D CT轮廓勾画方式。方法 :选取10例行胸部4D CT患者,每例都采用两种方式进行IGTV勾画。一种为所有时相逐层勾画后融合,定义为IGTV1;另一种为单套勾画,其余时相作为背景播放,定义为IGTV2。为了保证测试充分性,由十位医生完成全部病例勾画。对比两种方法所得IGTV的几何中心位置、体积大小、重合度及勾画时间。结果 :两种方式产生的IGTV几何中心在x、y、z轴上平均差异都小于1mm。IGTV1、IGTV2体积分别为(23.13±20.05)cm3、(23.27±19.86)cm3,体积比为1.02±0.04,两者重合度为96.05%±1.73%。两种勾画方式所产生IGTV几何中心位置和体积均无统计学意义差別,几何形状也重合较好。但采用本文探讨组织轮廓勾画方式,可显著缩短医生组织轮廓勾画时间,平均勾画时间从25.52min降至7.9min。结论 :本文探讨IGTV勾画方式勾画准确度完全能满足临床应用要求,且勾画速度提高了3倍。Objective: By comparing the two methods drawn internal gross tumor volume(IGTV) position and volume differences, explore a satisfy the requirement of clinical application and efficient contouring way of 4D CT. Methods: Select 10 cases of patients with chest 4D CT, each patient are used in two contouring ways of IGTV. One for fusion of all phase drawn contours, defined as IGTV1; another contouring of a single set, and the remaining phase as a background player, defined as IGTV2. In order to ensure adequate testing, asked ten doctors to finish all the cases outlined. Comparing the difference of the two methods resulted IGTV geometric center position, the volume size, coincidence degree and contouring time. Results: The mean difference in the geometric center in two ways of IGTV in x,y,z axis is less than 1mm.The volume of IGTV1 and IGTV2 were(23.13±20.05)cm3、(23.27±19.86)cm3, the volume ratio is 1.02 ± 0.04. The coincidence was 96.05%±1.73%. Changes of IGTV geometric center position of two kinds of contouring way are not obvious, geometrical shape is coincident well. But we proposed contouring way, can significantly shorten the doctor's contouring time. Average contouring time decreased from 25.52 minutes to 7.9 minutes. Conclusions: The accuracy of contouring way we proposed can completely satisfy the requirement of clinical application, and three times faster.
