病房降噪综合干预对新生儿生理指标及应激激素水平的影响

摘　　要：目的探讨病房降噪综合干预对新生儿生理指标及应激激素水平的影响。方法新生儿78例均分为干预组和对照组(未行干预)。干预组施行新生儿病房降噪综合干预措施,精密脉冲声级计24-h测量病房噪声(每3小时测量1次)。记录两组新生儿在09:00、15:00、23:00及次日05:00四个时段的体温、心率、呼吸和入院时、出院前的体重、肾上腺素、去甲肾上腺素水平。结果干预后环境噪声等效声级限值在夜间23:00-05:00达到国家标准。干预组新生儿23:00、次日05:00体温、心率、呼吸均低于09:00、15:00(P<0.05),且低于对照组(P<0.05)。干预组新生儿出院前体重增加(P<0.05),且肾上腺素、去甲肾上腺素水平降低(P<0.05或P<0.01),而对照组肾上腺素、去甲肾上腺素水平反而升高(P<0.05)。结论病房降噪后,新生儿病房环境更利于新生儿的身心发育。Objective To explore the effect of comprehensive intervention of ward noise reduction on physiological indicators and stress hormone in neonates.Methods Seventy-eight neonates were equally divided into two groups of A（treated with comprehensive intervention of ward noise reduction）and C（without intervention）.Ward noise was measured by impulse precision sound level meter for 24hours（once per 3hours）.Body temperature,heart rate and breath of neonates at 09：00,15：00,23：00and next day 05：00,body weight and serum levels of epinephrine and norepinephrine were measured.Results After intervention,noise equivalent sound level limit from 23：00at night to05：00in the morning reached national standard.Body temperature,heart rate and respiratory rate of neonates at 23：00and 05：00of next morning in group A were lower than those at 09：00and 15：00,which were also lower than those in group C（P〈0.05）.Body weight of neonates increased,and serum levels of epinephrine and norepinephrine decreased in group A before discharge（P〈0.05 or P〈0.01）,but serum levels of epinephrine and norepinephrine increased in group C before discharge（P〈0.05）.Conclusion After reduction of ward noise,neonatal ward environment is more beneficial to physical and mental development of neonates.
