环孢素A治疗特发性膜性肾病的Meta分析

摘　　要：目的系统评价环孢素A(CsA)治疗特发性膜性肾病(IMN)的疗效和安全性。方法电子检索Cochrane图书馆、PubMed、Medline database和中国期刊全文数据库、中国生物医学文献数据库、维普数据库、万方数据库,收集环孢素A治疗IMN的随机对照临床试验(RCTs),检索时间截止2014年10月25日,对符合标准的文献用Rev Man5.3进行Meta分析。结果共纳入国内外6个随机对照研究,合计302例患者。Meta分析结果显示,环孢素A治疗组与对照组相比,治疗IMN的总缓解率无明显差别(P=0.13),复发率差异亦无统计学意义(P=0.43)。结论目前证据表明,环孢素A治疗IMN与传统的治疗方案相比总体疗效相当,对于不能耐受细胞毒药物不良反应的患者,可作为替代治疗方案。Objective To assess the efficacy and safety of cyclosporine in the treatment of idiopathic membranous nephropathy（IMN）.Methods Cochrane Library,Pub Med,EMBASE,CNKI,CBM and VIP,wanfang were searched for the randomized controlled clinical trials（RCTs） involved in the treatment of idiopathic membranous nephropathy by cyclosporine before October 25,2014.The studies which met the included criteria were included and a Meta-analysis was performed by Review Manager 5.3.Results 6 studies were included,involving 302 patients.The results of Meta analysis showed that there was no significant difference in the incidence of complete remissions（P =0.13） and relapse（P = 0.43） when compared with the control group respectively.Conclusion The efficacy of cyclosporine in the treatment of idiopathic membranous nephropathy is the same as the traditional protocols.Cyclosporine can be the alternative treatment options for the patients who could not tolerate cytotoxic drug adverse reaction.
