金蝉保肾汤对慢性肾炎患者蛋白尿疗效观察

摘　　要：目的:探讨金蝉保肾汤对原发性慢性肾小球肾炎尿蛋白的疗效。方法:选取我院2009—2013年原发性慢性肾小球肾炎(PCGN)患者共58例。将58例病人随机分为治疗组29例,用金蝉保肾汤1剂分3袋,每次1袋100 m L,每日3次口服。对照组29例,依那普利10 mg每日1次,黄葵胶囊3粒,每日3次口服。疗程为2个月。观察指标:(1)尿常规蛋白,24 h尿蛋白(UP)定量;(2)血肌酐(Cr),尿素氮(BUN),血浆白蛋白(Alb),甘油三酯(TG),低密度脂蛋白(LDLC),胆固醇(CHO)。结果:(1)两组尿蛋白显效率、有效率比较,与对照组比较,显效率P<0.05,差异有统计学意义,总有效率P<0.01,有显著差异。(2)治疗前后同组相比及治疗后金蝉保肾汤组与对照组相比,24 h尿蛋白、血肌酐、血尿素氮、血浆清蛋白水平存在显著差异(P<0.05)。结论:金蝉保肾汤明显减少慢性肾炎患者的尿蛋白,提高血浆白蛋白,其疗效明显优于依那普利、黄葵胶囊;对延缓慢性肾功能不全的进展,金蝉保肾汤也明显优于依那普利。Objective：Study Jinchan Baoshen Tang on the curative effect of primary chronic glomerulonephritis urine protein. Methods：Select 58 patients with primary chronic glomerulonephritis（PCGN）of our hospital from 2009 to 2013. The 58 patients were randomly divided into group A and group B. The treatment group：29 cases,using Jinchan Baoshen Tang one bag of 100 m L,three times adjective per day. Control group：29 cases,enalaprilat 10 mg once daily,ambrette capsules 3 pills,three times adjective per day. Treatment for 2 months. Observation indexes：（1）Urine protein,24 hours urinary protein（UP）the quantitative;（2）Serum creatinine（Cr）,urea nitrogen（BUN）,serum albumin（Alb）,triglycerides（TG）,low density lipoprotein（LDLC）,cholesterol（CHO）. Results：（1）Two significant efficiency urine protein,a relatively efficient,compared with the control group,significant efficiency P〈0.05,the difference was statistically significant. Total efficiency P0.01 significantly different.（2）Compared to the same group before and after treatment by Jinchanbaoshentang. Compared the treatment group by Jinchan Baoshen Tang with the control group. In 24 hours,the urinary protein,serum creatinine,blood urea nitrogen,serum albumin level exists significant difference（P〈0.05）.Conclusion：Jinchan Baoshen Tang significantly reducing urinary protein in patients with chronic nephritis and improve plasma albumin. its curative effect was better than enalaprilat,ambrette capsule;To slow down the progress of chronic renal insufficiency is obviously better than that of enalaprilat.
